绝经后女性高血压患者性激素结合球蛋白、性激素与LP-PLA2、A-FABP相关性研究

Correlation between sex hormone binding globulin and sex hormones and LP-PLA2 and A-FABP in postmenopausal women with hypertension

目的探讨绝经后女性高血压患者血清炎症标志物脂蛋白相关磷脂酶A2(Lp-PLA2)、脂肪细胞性脂肪酸结合蛋白(A-FABP)水平与性激素结合球蛋白(SHBG)、总睾酮(T)和雌二醇(E2)的相关性。方法连续入选2014年8月至2015年10月于青岛市第八人民医院门诊就诊及住院的老年原发性女性高血压患者235例(均为绝经≥5年),测定所有观察对象血清Lp-PLA2和A-FABP及SHBG、T和E2水平,使用层级回归分析模型探索SHBG、T、E2与Lp-PLA2和A-FABP之间的关系。结果在控制年龄、体重指数(BMI)、腰臀比(WHR)、收缩压(SBP)、舒张压(DBP)、空腹血糖(FPG)、总胆固醇(TC)、三酰甘油(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、空腹胰岛素(RI)、高敏C反应蛋白(hs-CRP)对SHBG的影响后,Lp-PLA2与SHBG成负相关(-0.262±0.126,P=0.040),A-FABP(-0.012±0.040,P=0.766)和SHBG没有显著相关性。Lp-PLA2(-0.013±0.053,P=0.806)、A-FABP(-0.006±0.017,P=0.734)和E2均没有显著相关性。Lp-PLA2(-0.063±0.130,P=0.630)、A-FABP(-0.010±0.041,P=0.812)与T均没有显著相关性。结论在无激素替代治疗的绝经后女性高血压患者,性激素结合球蛋白与脂蛋白相关磷脂酶A2成负相关,总睾酮和雌二醇与脂蛋白相关磷脂酶A2无明显相关性,脂肪细胞性脂肪酸结合蛋白与性激素结合球蛋白、总睾酮和雌二醇均无明显相关性。

Objective To investigate the correlation of serum inflammatory markers lipoprotein related phospholipase A2(Lp-PLA2)and fatty acid binding protein(A-FABP)level with sex hormone binding globulin(SHBG),total testosterone(T)and estradiol(E2)in patients with postmenopausal hypertension.Method The study included 235 elderly patients with primary hypertension who were treated and hospitalized in the outpatient department of Qingdao Eighth People's Hospital from August 2014 to October 2015(both menopause≥5 years).Serum Lp-PLA2 was measured in all subjects.And the A-FABP and SHBG,T and E2 levels,using a hierarchical regression analysis model to explore the relationship between SHBG,T,E2 and Lp-PLA2 and A-FABP.Result Controlling age,body mass index(BMI),waist-to-hip ratio(WHR),systolic blood pressure(SBP),diastolic blood pressure(DBP),fasting blood glucose(FPG),total cholesterol(TC),triglyceride(TG),high density Lp-PLA2 is negatively correlated with SHBG after the effects of lipoprotein cholesterol(HDL-C),low-density lipoprotein cholesterol(LDL-C),fasting insulin(RI),and high-sensitivity C-reactive protein(hs-CRP)on SHBG(-0.262±0.126,P=0.040),A-FABP(-0.012±0.040,P=0.766)and SHBG had no significant correlation.There was no significant correlation between Lp-PLA2(-0.013±0.053,P=0.806),A-FABP(-0.006±0.017,P=0.734)and E2.Lp-PLA2(-0.063±0.130,P=0.630),A-FABP(-0.010±0.041,P=0.812)had no significant correlation with T.Conclusion In postmenopausal women with hypertension without hormone replacement therapy,sex hormone binding globulin is negatively correlated with lipoprotein associated phospholipase A2;total testosterone and estradiol had no correlation with lipoprotein associated phospholipase A2;there was no correlation between adipocyte fatty acid binding protein and sex hormone binding globulin,total testosterone and estradiol.
