消痈散结膏外敷联合穿刺后隔姜灸治疗急性乳腺脓肿的临床研究

Clinical Observation of Xiaoyong Sanjie Ointment Combined with Ginger Moxibustion on Acute Mammary Abscess

目的观察消痈散结膏外敷联合穿刺后隔姜灸治疗急性乳腺脓肿的临床疗效。方法选择明确诊断急性乳腺脓肿患者150例,随机分为5组,均予以抗菌药物治疗,按疗程分为急性期、成脓期、愈合期3段时期,第1～4组为实验组,第1组急性期予以中药外敷、成脓期穿刺抽液后,愈合期予隔姜灸消除肿块;第2组急性期不予以处理,成脓期予穿刺抽液后,愈合期隔姜灸消除肿块;第3组急性期予中药外敷,成脓期穿刺抽液后,愈合期不予处理;第4组急性期、愈合期均不予以处理,仅成脓期予穿刺抽液后;第5组为对照组,急性期、愈合期不予中医处理,成脓期予以切开排脓术治疗。通过比较5组患者局部红肿热痛症状消失时间、乳房肿块消散时间、治疗总时间,抗菌药物使用时间,治疗时疼痛评分,回乳率等指标,评定疗效。结果治疗后,第1组局部红肿热痛症状消失时间、乳房肿块消散时间、抗菌药物使用时间、治疗总时间均最短,患者主观痛苦最少,回乳率最低(均P <0.05)。结论消痈散结膏外敷联合穿刺后隔姜灸治疗急性乳腺脓肿疗效满意,临床上有广泛应用前景。

Objective: To observe the clinical curative effect of the treatment of acute mastitis in the treatment of acute mastitis. Methods: 150 cases were randomly divided into 5 groups. All patients were treated with antibiotics. According to the course of treatment,they were divided into three phases: acute phase,non-acute phase,and healing phase. The first to fourth groups were experimental groups. The fifth group was the control group. According to different treatment course,corresponding treatment was given to each group. The clinical efficiency was evaluated by comparing the disappearance time of inflammation,the dissipation time of induration,total treatment time,antibiotic use time,pain score during treatment,the rate of breast milk disappearing and other indicators. Results: After treatment,the first group had the shortest disappearance time of local swelling,fever and pain symptoms,breast mass dissipation time,antibiotic use time and total treatment time,with the lowest subjective pain and the lowest rate of breast regurgitation. Conclusion: The clinical curative effect of Xiaoyong Sanjie Ointment external application combined with ginger moxibustion after puncture on acute mastitis is sanctifying.
