临床路径管理对早期胃癌根治术后患者胃肠道功能恢复及并发症的影响

Effect of clinical pathway management on gastrointestinal function recovery and complications in patients with early gastric cancer after radical gastrectomy

目的探讨临床路径管理对早期胃癌根治术后患者胃肠道功能恢复及并发症的影响。方法根据临床管理办法的不同将120例行胃癌根治术的早期胃癌患者分为常规组和观察组,每组60例。比较两组患者的并发症发生情况、胃肠道功能恢复相关指标及生存情况。结果常规组患者的并发症总发生率为25.0%,高于观察组患者的10.0%(P﹤0.05)。术后,观察组患者的开始进食时间、肠鸣音出现时间、首次排气时间、首次排便时间均明显短于常规组(P﹤0.01)。观察组患者的生存情况优于常规组(P﹤0.05)。结论临床路径管理干预能够明显改善早期胃癌根治术后患者并发症的发生情况,缩短胃肠道功能恢复时间,延长患者的生存时间。

Objective To study the effect of clinical pathway management on the recovery of gastrointestinal function and the impact on occurrence of complications after radical gastrectomy for early gastric cancer.Method According to different clinical management,120 patients with early gastric cancer after radical gastrectomy were assigned as routine care group and study group,respectively,with 60 patients in each.The occurrence of complications,indicators of gastrointestinal function recovery and survival were compared between the two groups.Result The overall incidence rate of complications in the routine care group was 25.0%,and it was higher than the 10.0% in study group (P<0.05).The time to oral feeding,recovery time of bowel sounds,time to first exhaust,and the time to first defecation after operation were significantly shorter in the study group than those in the routine care group,and the differences were of statistical significance (P<0.01).The survival of the study group was better than that of the routine care group (P<0.05).Conclusion The intervention of clinical pathways management in patients with early gastric cancer after radical gastrectomy can significantly improve the occurrence of complications,and shorten the time needed for gastrointestinal function recovery,with prolonged survival time.
