改良法治疗急性泪囊炎导致前部眶蜂窝织炎的临床观察

An improved treatment on anterior orbital cellulitis caused by acute dacryocystitis

目的分析改良法和常规法治疗急性泪囊炎导致前部眶蜂窝组织炎的疗效。方法将2016年3月至2017年11月在本院门诊就诊的急性泪囊炎合并前部眶蜂窝组织炎的患者共26例(26只眼),分为改良组13例(13只眼)和常规组13例(13只眼)。改良组用全身广谱抗生素抗感染联合急性期经上泪小管引流抽吸泪囊内脓液、泪囊内注药;常规组予以全身广谱抗生素治疗,待感染局限后采取经皮肤穿刺抽取泪囊内脓液。观察2组治疗效果,包括治疗时间长短、治疗综合效果得分。结果改良组较常规组治疗时间明显缩短,治疗综合效果得分高于常规组,均有统计学意义(P<0.05)。结论全身广谱抗生素抗感染联合急性期经上泪小管引流抽吸泪囊内脓液、泪囊内注药的方法治疗急性泪囊炎导致的眼部眶蜂窝组织炎,临床疗效较好。

OBJECTIVE To evaluate effects of an improved method and the conventional method on anterior orbital cellulitis caused by acute dacryocystitis. METHODS Twenty-six patients diagnosed as anterior orbital cellulitis caused by acute dacryocystitis in our hospital from March 2016 to November 2017 were evenly divided into two groups and were studied. In improved surgery group, broad spectrum antibiotic was used for infection, at the same time, the pus was drained through the upper lacrimal duct and drug was injected into the lacrimal sac. In conventional surgery group, broad spectrum antibiotic was used to resist infection, until the infection was in control, the pus was pumped by puncture. The results(treatment duration, comprehensive score of treatment) of two groups were observed. RESULTS The treatment time of the improved surgery group was shorter than that of the conventional group(P<0.05).The comprehensive scores of improved treatment group were higher than that of the conventional treatment group(P<0.05).CONCLUSIONS The improved therapeutic plan was a more scientific solution of more favorable effects on anterior orbital cellulitis caused by acute dacryocystitis in contrast to the conventional method.
