2型糖尿病合并急性脑梗死患者左心室功能变化 被引量:5

Change of left ventricular function in type 2 diabetes mellitus patients complicated with acute cerebral infarction

摘要 目的比较单纯2型糖尿病患者、单纯急性脑梗死患者和2型糖尿病合并急性脑梗死患者的左心室功能。方法纳入单纯2型糖尿病患者(糖尿病组)、单纯急性脑梗死患者(脑梗死组)、2型糖尿病合并急性脑梗死患者(合并组)各50例。所有患者均行心脏彩超检查测定左心室收缩功能指标(心脏指数、射血分数和短轴缩短率)、左心室舒张功能指标[等容舒张时间(IVRT)、E/A比值和E波减速时间(EDT)]和左心室整体功能指标(Tei指数)。结果 3组患者左心室收缩功能指标差异无统计学意义(P> 0. 05)。与其他两组比较,合并组IVRT、EDT长,Tei指数高,E/A比值低(均P <0. 05),但糖尿病组和脑梗死组间差异无统计学意义(P> 0. 05)。结论与单纯2型糖尿病或急性脑梗死患者相比,2型糖尿病合并急性脑梗死患者左心室整体功能降低,这可能与其左心室舒张功能降低有关。 Objective To compare left ventricular function among patients with simple type 2 diabetes mellitus,simple acute cerebral infarction,and type 2 diabetes mellitus complicated with acute cerebral infarction.Methods Fifty simple type 2 diabetes mellitus( diabetic group),50 simple acute cerebral infarction( cerebral infarction group) and 50 type 2 diabetes mellitus complicated with acute cerebral infarction( complication group) were enrolled.All the patients received color Doppler echocardiography to measure their relevant indices,including left ventricular systolic function indices( cardiac index,ejection fraction,and fractional shortening rate),left ventricular diastolic function indices[isovolumic relaxation time( IVRT),E/A ratio,and E-wave deceleration time( EDT)],and global left ventricular function index( Tei index). Results There were no statistically significant differences in left ventricular systolic function indices among the three groups( P > 0. 05). Compared with the other two groups,the complication group had prolonged IVRT and EDT,increased Tei index,and decreased E/A ratio( all P < 0. 05),but no statistically significant differences were found between the diabetic group and the cerebral infarction group( P > 0. 05). Conclusion Compared with patients with simple type 2 diabetes mellitus or simple acute cerebral infarction,type 2 diabetes mellitus patients complicated with acute cerebral infarction experience decreased global left ventricular function,which probably results from decreased left ventricular systolic function.
