摘要
目的观察人工关节置换术联合碳酸钙及鲑鱼降钙素治疗老年股骨粗隆间骨折的临床效果。方法选择2016年1月—2018年2月收治的老年股骨粗隆间骨折125例。根据治疗方法分为观察组65例和对照组60例。2组均给予人工髋关节置换术,对照组于术前1周开始给予碳酸钙D3,观察组在对照组基础上给予皮下注射鲑鱼降钙素。观察2组手术情况;比较2组治疗前后Harris评分及SF-36生活质量评分,视觉模拟疼痛评分(VAS)评估疼痛程度;检测2组治疗前后骨钙素(OC)、骨碱性磷酸酶(BALP)和骨密度(BMD)水平。结果观察组骨折愈合时间短于对照组(P <0. 05)。观察组治疗后Harris、SF-36、VAS评分、OC、BALP、BMD改善情况优于对照组(P <0. 05)。结论鲑鱼降钙素用于人工关节置换术后老年患者可以有效抑制骨质疏松发展,缓解术后疼痛,促进关节功能恢复和改善生活质量。
Objective To observe clinical effect of artificial joint replacement combined with salmon calcitonin in treatment of elderly patients with fractures of femurs haft. Methods A total of 125 elderly patients with fractures of femurs haft admitted during January 2016 and February 2018 were divided into observation group ( n =65) and control group ( n =60) according to therapeutic methods. All patients underwent artificial joint replacement. Control group was given calcium carbonate (D3) in 1 week before operation, while observation group was given subcutaneous injection of salmon calcitonin on the basis of treatment for control group. In two groups, operative conditions were observed;Harris score and SF-36 life quality score before and after treatment were compared;visual analogue scale (VAS) was used to assess pain degree. Osteocalcin (OC), bone alkaline phosphatase (BALP) and bone mineral density (BMD) levels were detected before and after treatment. Results Healing time of fractures in observation group was shorter than that in control group ( P <0.05). Harris, SF-36 and VAS scores, and improved degrees of OC, BALP and BMD in observation group were better than those in control group ( P <0.05). Conclusion Salmon calcitonin in application of elderly patients after artificial joint replacement may effectively inhibit development of osteoporosis, relieve postoperative pain, promote recovery of joint function and improve quality of life.
