不同分期子宫内膜异位症行血清脑源性神经营养因子检测的临床价值 被引量:4

摘要 目的探讨不同分期子宫内膜异位症患者行血清脑源性神经营养因子(BDNF)检测的临床价值。方法回顾性选取中国石油天然气总公司中心医院2017年1月至2018年10月收治的134例子宫内膜异位症患者作为观察组,入选者均经腹腔镜手术及病理证实;根据子宫内膜异位症分期标准进行分期:Ⅰ期29例,Ⅱ期40例,Ⅲ期45例,Ⅳ期20例;根据痛经分级标准分组:轻度38例,中度57例,重度39例。另选取同期来院体检健康女性85例为对照组。入组者均采用酶联免疫吸附试验(ELISA)检测血清BDNF、糖类抗原-125(CA125)及超敏C反应蛋白(hs-CRP)水平。结果观察组的BDNF、CA125及hs-CRP水平均显著高于对照组(P <0. 05)。随着r-AFS分期的加重,患者的BDNF及CA125水平呈升高趋势(P <0. 01)。Ⅱ期的BDNF水平高于Ⅰ期,差异有显著性(P <0. 05),而其CA125及hs-CRP水平比较差异无显著性(P> 0. 05)。不同痛经严重程度各组BDNF、CA125水平比较,差异有显著性(P <0. 01),中度组及重度组的BDNF、CA125水平明显高于轻度组(P <0. 05)。结论早期子宫内膜异位症患者BDNF水平显著升高,其在预测早期病变方面有较大潜力,联合CA125及hs-CRP检测有望为临床早期诊断提供更有力依据。 Objective To investigate the clinical value of serum brain-derived neurotrophic factor(BDNF)in patients with endometriosis at different stages.Methods 134 patients with endometriosis admitted in our hospital from January 2017 to October 2018 were selected as the observation group.All the patients were confirmed by laparoscopy and pathology.Another 85 healthy women who were admitted to the hospital during the same period were selected as control group.The serum levels of BDNF,CA125 and hs-CRP were detected by enzyme-linked immunosorbent assay(ELISA).Results The levels of BDNF,CA125 and hs-CRP in the observation group were significantly higher than those in the control group(P<0.05).With the aggravation of r-AFS staging,the levels of BDNF and CA125 in the patients increased(P<0.01).There was a significant difference in BDNF level between stage II and stage I(P<0.05).There was no significant difference in CA125 and hs-CRP levels between the two groups(P>0.05).There were significant differences in BDNF and CA125 levels among different menstrual severity groups(P<0.01).The levels of BDNF and CA125 in moderate group and severe group were significantly higher than those in mild group(P<0.05).Conclusion The level of BDNF in patients with early endometriosis is significantly higher,which has great potential in predicting early lesions.Combined detection of CA125 and hs-CRP may provide a more powerful basis for early clinical diagnosis.
