PCOS伴或不伴高胆固醇血症患者行体外授精胚胎指标差异分析

Embryonic parameters in polycystic ovary syndrome patients with and without hypercholesterolemia in IVF

目的比较多囊卵巢综合征(PCOS)伴和不伴高胆固醇血症患者行体外授精(IVF)助孕的胚胎指标差异。方法选择PCOS患者87例和非PCOS患者263例。以血清总胆固醇(COH)值是否≥5.2 mmol/L将PCOS患者分为伴高胆固醇血症组(C+PCOS组)和不伴高胆固醇血症组(C-PCOS组),非PCOS患者组分为伴高胆固醇血症组(C+对照组)和不伴高胆固醇血症组(C-对照组),比较各组在回收卵率、2PN率、优质卵裂胚比率、优质囊胚比率等胚胎指标的差异。结果 C+PCOS组较C-PCOS组回收卵率和优质卵裂胚比率显著降低(P<0.05,2组间2PN率和优质囊胚比率差异无统计学意义(P>0.05);C+对照组较C-对照组回收卵率显著降低(P<0.05,2组间2PN率、优质卵裂胚比率和优质囊胚比率差异无统计学意义(P>0.05);PCOS组发生高胆固醇血症的比例较对照组发生高胆固醇血症的比例升高,但两者无统计学差异(P>0.05)。结论 PCOS伴高胆固醇血症患者较不伴高胆固醇血症患者回收卵率和卵裂胚质量显著下降,非PCOS患者伴高胆固醇血症与回收卵率下降相关。

Objective To compare the differences of embryo parameters between polycystic ovary syndrome (PCOS) with or without hypercholesterolemia in vitro fertilization (IVF).Methods 87 patients with PCOS were selected and 263 non-PCOS patients served as controls.According to the serum total cholesterol (COH) value was higher than 5.2 mmol/L or not,PCOS patients were divided into with hypercholesterolemia group (C+ PCOS group) and without hypercholesterolemia group (C- PCOS group).Non-PCOS patients were divided into with hypercholesterolemia group (C+ Control group) and without hypercholesterolemia group (C- Control group). The differences were compared in ovum recovery rate and 2PN rate,high quality embryo rate and high quality blastocyst rate of each group.Results Comparing to C-PCOS group,ovum recovery rate and high quality embryo rate decreased significantly in the C+ PCOS group (P < 0.05),2PN rate and high quality blastocyst rate shew no significant differences in 2 groups (P >0.05);In non-PCOS patients,ovum recovery rate was significantly decreased (P < 0.05) in C + control group comparing with C- control group.2PN rate,high quality embryo rate and high quality blastocyst rate shew no significant differences in 2 groups (P >0.05).The proportion of hypercholesterolemia in group PCOS was higher than that in control group,but there was no significant difference between the 2 groups (P>0.05). Conclusions In PCOS patients with hypercholesterolemia,the ovum recovery rate and the quality of cleavage embryos decreased significantly than that in patients without hypercholesterolemia,whereas non-PCOS patients with hypercholesterolemia were associated with decreasing ovum recovery rate.
