消化性溃疡患者根除HP后发生胃食管反流病的研究

Study of Gastroesophageal Reflux Disease after HP Eradication in Patients with Peptic Ulcer

目的:观察消化性溃疡患者根除幽门螺杆菌(HP)后发生胃食管反流病的情况,探讨HP与胃食管反流病(GERD的关系。方法:将60例HP阳性消化性溃疡患者随机分为观察组和对照组,各30例,观察两组患者总体疗效,GerdQ评分,HP根除情况及两组患者治疗前、后及停药3个月胃食管反流发生情况。结果:对照组总有效率明显低于观察组(P<0.05);对照组GerdQ评分明显低于观察组(t=2.243,P<0.05);观察组HP根除率高于对照组(χ2=7.2,P<0.05);治疗后,在消化性溃疡患者发生胃食管反流病方面,观察组与对照组差异无统计学意义(χ2=0.3,P>0.05);停药3月后,观察组有3例消化性溃疡患者发生胃食管反流病,对照组有5例,差异无统计学意义(χ2=0.577,P>0.05)。结论:消化性溃疡患者经根除HP后效果显著,HP与胃食管反流病的发生可能不存在相关性。

Objective: To observe the occurrence of gastroesophageal reflux disease(GERD) after HP eradication in patients with peptic ulcer. Methods: 60 patients with helicobacter pylori(HP) positive peptic ulcer were randomly divided into the observation group and the control group, 30 patients each group. The overall efficacy, GerdQ score, HP eradication, and the occurrence of gastroesophageal reflux before, after and after treatment for 3 months were observed. Results: The total effective rate of the control group was significantly lower than that of the observation group(P<0.05). The Gerd Q score of the control group was significantly lower than that of the observation group(t=2.243, P<0.05). The eradication rate of HP in the observation group was higher than that in the control group(χ^2=7.2, P<0.05). After treatment, there was no statistically significant difference between the observation group and the control group in the occurrence of gastroesophageal reflux disease in patients with peptic ulcer(χ^2=0.3, P>0.05). Three patients with peptic ulcer developed gastroesophageal reflux disease in the observation group and 5 patients in the control group after 3 months of drug withdrawal,with no statistically significant difference(χ^2=0.577, P>0.05). Conclusion: The effect of HP eradication on peptic ulcer was significant,and there was no correlation with the occurrence of gastroesophageal reflux.
