甲泼尼龙联合治疗方案治疗难治性肺炎支原体肺炎患儿的临床效果

Clinical effect of Methylprednisolone combined therapy programs in the treatment of refractory Mycoplasma pneumoniae pneumonia in children

目的探讨甲泼尼龙联合治疗方案治疗难治性肺炎支原体肺炎患儿的临床效果。方法选择2015年1月～2017年12月大连市中心医院收治的86例难治性肺炎支原体肺炎患儿作为研究对象,按照随机数字表法将其分为观察组和对照组,每组各43例。对照组患儿给予阿奇霉素、头孢他啶等治疗,观察组患儿在对照组的治疗基础上联用甲泼尼龙。观察两组患儿的治疗效果、临床症状缓解、血清C反应蛋白(CRP)、乳酸脱氢酶(LDH)水平及红细胞沉降率(ESR)。结果观察组患儿的临床总有效率为93.0%,明显高于对照组的67.4%,差异有统计学意义(P<0.05)。观察组患儿的咳嗽消失时间、高热消失时间、肺部啰音消失时间、肺部阴影消失时间、住院时间均明显短于对照组,差异有统计学意义(P<0.01)。观察组患儿治疗后的CRP、LDH水平及ESR均明显低于对照组,差异有统计学意义(P<0.01)。结论甲泼尼龙联合治疗方案治疗难治性肺炎支原体肺炎患儿的临床效果确切,能显著缓解临床症状及炎症反应。

Objective To explore the clinical effect of Methylprednisolone combined therapy programs in the treatment of refractory Mycoplasma pneumoniae pneumonia in children.Methods A total of 86 children with refractory Mycoplasma pneumoniae pneumonia admitted to Dalian Central Hospital from January 2015 to December 2017 were selected as subjects and divided into the observation group and the control group according to random number table method,43 cases in each group.The control group was treated with Azithromycin and Ceftazidime,while the observation group was treated with Methylprednisolone on the basis of the control group.The therapeutic effect,clinical symptoms,the levels of serum C reactive protein(CRP),lactate dehydrogenase(LDH)and erythrocyte sedimentation rate(ESR)of the two groups were observed.Results The total clinical effective rate in the observation group was 93.0%,which was significantly higher than that in the control group(67.4%),and the difference was statistically significant(P<0.05).In the observation group,the time of disappearance of cough,the time of high fever disappearance,the time of disappearance of pulmonary rales,the time of disappearance of lungs,and the time of hospital stay were significantly shorter than those in the control group,and the differences were statistically significant(P<0.01).After treatment,the CRP level,the LDH level and the ESR of the observation group were significantly lower than those in the control group,and the differences were statistically significant(P<0.01).Conclusion The combination therapy of Methylprednisolone in the treatment of refractory Mycoplasma pneumoniae pneumonia is effective,which can alleviate the clinical symptoms,relieve the inflammatory reaction and promote the rehabilitation of the disease.
