预见性干预对肾结石患者微创经皮肾镜碎石术后并发症的预防价值

Study on the Value of Predictive Intervention for Prevention of Complications in Patients with Renal Calculi after Minimally Invasive Percutaneous Nephrolithotomy

目的探究预见性干预对肾结石患者微创经皮肾镜碎石术(percutaneous nephrolithotripsy,PCNL)后并发症的预防价值。方法将96例行PCNL治疗的肾结石患者随机分为观察组和常规组各48例。观察组采取预见性干预,常规组行基础护理。比较两组患者术后并发症发生率和舒适度差异,观察手术前后患者不良情绪评分变化情况。结果观察组患者术后并发症发生率8. 33%低于常规组22. 92%,简化舒适状况量表(GCQ)评分(91. 23±15. 47)分则高于常规组(72. 58±16. 25)分,差异均有统计学意义(P﹤0. 05)。术后观察组患者焦虑自评量表(SAS)评分(39. 75±9. 63)、抑郁自评量表(SDS)评分(40. 32±9. 41)分,常规组患者焦虑自评量表(SAS)评分(45. 12±10. 36)、抑郁自评量表(SDS)评分(46. 74±9. 25)分均较术前降低,且观察组低于常规组,差异均有统计学意义(P﹤0. 05)。结论预见性干预可减少肾结石患者PCNL术后并发症的发生,且能安抚患者不良情绪,提高舒适度。

Objective To explore the value of predictive intervention for prevention of complications in patients with renal calculi after minimally invasive percutaneous nephrolithotomy(PCNL).Methods 96 cases of renal calculi treated with PCNL were randomly divided into observation group and conventional group,with 48 cases in each group.The observation group received predictive intervention,while the conventional group was given basic nursing.The incidence of postoperative complications and the comfort degree were compared between the two groups,as well as the changes of patients'bad mood scores.Results The incidence of postoperative complications in the observation group was 8.33%,lower than that in the conventional group(22.92%),and GCQ score(91.23+15.47)was higher than that in the conventional group(72.58+16.25).The difference was statistically significant(P<0.05).SAS score(39.75+9.63),SDS score(40.32+9.41)in the observation group,SAS score(45.12+10.36)and SDS score(46.74+9.25)in the conventional group were all lower than those before operation,and those in the observation group was lower than those in the conventional group,with statistically significant difference(P<0.05).Conclusion Predictive intervention can reduce the incidence of complications in patients with renal calculi after PCNL,and what’s more,it can soothe patients'bad mood and improve their comfort degree.
