老年人睡眠障碍与衰弱关系的meta分析

A meta-analysis of association between sleep disorders and frailty in the elderly

目的:运用meta分析的方法评价老年人睡眠障碍与衰弱的关系。方法:检索Pubmed、MEDLINE、CENTRAL、Embase、Scopus、CINAHL、中国知网、中国生物医学文献数据库、维普和万方数据库,检索时限均为建库至2018年7月,收集相关研究。两名研究者独立筛选文献、提取资料并评价纳入研究质量后,采用RevMan5.3软件进行meta分析。结果:纳入10个横断面研究,共7598例患者。未校正OR[OR=2.70,95%CI(1.98,3.67),P<0.001]和校正OR[OR=1.30,95%CI(1.11,1.52),P<0.01]均显示,老年人睡眠障碍与衰弱呈正相关。亚组分析结果显示,不同睡眠障碍类型[失眠、嗜睡、睡眠呼吸障碍及其他睡眠问题(OR=2.79、2.30、1.35、2.65)],性别[男性(OR=1.33)、女性(OR=2.21)],研究场所[医院(OR=3.49)、社区(OR=2.08)],研究地区[北美洲(OR=1.31)、亚洲(OR=3.18)]老年人睡眠障碍均与衰弱相关(均P<0.05)。结论:老年人睡眠障碍与衰弱的发生相关。

Objective: To assess the associations between sleep disorders and frailty in the elderly by metaanalysis. Methods: Pubmed,MEDLINE,CENTRAL,CINAHL,Embase,Scopus,CNKI,CBM,VIP and WanFang Data were searched from inception to July 2018 to collect related observational studies. According to the inclusion and exclusion criteria,two researchers screened the articles,extracted the data,and assessed the quality of the studies independently,meta-analysis was performed using RevMan 5. 3 software. Results: A total of 10 cross-sectional studies including 7598 participants were included. Meta-analysis showed that sleep disorders were associated with frailty in the elderly through crude OR [OR = 2. 70,95% CI ( 1. 98,3. 67),P < 0. 001] and adjusted OR [OR = 1. 30,95%CI ( 1. 11,1. 52),P < 0. 01]. Besides,subgroup analyses showed that insomnia, drowsiness,sleep disorders breathing,other sleep problems,male or female sleep disorders,sleep disorders at hospital or community,north American or Asian sleep disorders were all associated with frailty ( OR = 1. 31 - 3. 49,Ps < 0. 05). Conclusion: Sleep disorders are associated with frailty in the elderly.
