循环肿瘤细胞与喉鳞状细胞癌患者预后的关系

Relationship between circulating tumor cell and prognosis of laryngeal squamous cell carcinoma

目的初步探讨喉鳞状细胞癌(LSCC)患者外周血循环肿瘤细胞(CTC)的临床意义及其与患者预后的关系。方法将40例病理确诊为LSCC的患者被纳入本次研究,用Ad11-5ETel-GFP病毒检测外周血CTC,随访21～24个月,分析外周血CTC检测结果与各项临床参数及无进展生存期(PFS)的关系。结果 40例LSCC患者检测结果显示,CTC阳性组和阴性组年龄、肿瘤部位、TNM分期比较,差异无统计学意义(P>0.05);CTC阴性组和阳性组患者的PFS生存曲线比较,差异有统计学意义(P<0.05)。对LSCC患者复发影响因素进行分析,多变量Cox比例风险回归分析表明,有无淋巴结转移(RR=18.710,95%CI:2.394～146.224,P=0.005)、CTC(RR=13.302,95%CI:1.173～150.788,P=0.037)与患者局部复发有关。结论 CTC与LSCC患者是否复发有关,无CTC的患者预后更好。CTC与LSCC患者的年龄、部位、TNM分期无关。

Objective To investigate the prognostic significance of circulating tumor cell(CTC)in patients with laryngeal squamous cell carcinoma(LSCC).Methods A total of 40 patients with pathologically diagnosed LSCC were enrolled.CTC was detected by Ad11-5ETel-GFP.All patients were follow-up for 21 to 24 months.Correlation of CTC with age,sexuality,tumor location and TNM staging in patients with LSCC were statistically analyzed.Results There was no significant difference in age,tumor location and TNM stage between CTC negative group and positive group(P>0.05).While the progression-free survival curve of the CTC negative group and positive group was significantly different(P<0.05).Significant predictors for the recurrence of LSCC were analyzed.Multivariate Cox proportional hazards regression analysis indicated that lymph node metastasis(RR=18.710,95%CI:2.394-146.224,P=0.005)and CTC(RR=13.302,95%CI:1.173-150.788,P=0.037)were associated with local recurrence.Conclusion CTC plays an important role in the recurrence process of LSCC.Patients without CTC may mean better prognosis.There is no association between CTC and the age,sex,location or TNM stage of patients with LSCC.
