摘要 背景:C型凝集素结构域家族4成员D(CLEC4D)在系统性真菌感染中可诱导Th17细胞分化,调控IL-17A表达,目前国内外尚未见其在肠道免疫中是否发挥作用的报道。目的:探讨CLEC4D、CARD9在炎症性肠病(IBD)患者结肠组织中的表达情况及其临床意义。方法:收集2016年10月—2018年6月郑州大学第二附属医院收治的IBD患者48例,其中溃疡性结肠炎(UC)患者36例,克罗恩病(CD)患者12例,以30例结肠癌旁正常组织作为对照。采用免疫组化法检测结肠组织中CLEC4D和CARD9表达,并分析其与临床特征的相关性。结果:UC组、CD组结肠组织中CLEC4D和CARD9表达均显著高于对照组(P <0. 05),而UC组与CD组之间无明显差异(P>0. 05)。UC组CLEC4D、CARD9表达与Mayo评分、Baron分级呈正相关(P <0. 05),CD组CLEC4D、CARD9表达与CDAI评分呈正相关(P <0. 05),与SES-CD评分无明显相关性(P> 0. 05);IBD患者CLEC4D、CARD9表达与CRP呈正相关(P <0. 05),而与ESR无明显相关性(P> 0. 05)。结论:CLEC4D、CARD9在IBD患者中的表达升高,且与患者病情密切相关,CLEC4D可能通过CARD9相关途径参与IBD的肠道免疫。 Background:C-type lectin domain family 4 member D(CLEC4D)can induce Th17 cell differentiation and regulate IL-17A expression in systemic fungal infection,and whether CLEC4D plays a role in intestinal immunity has not been reported at home and abroad.Aims:To investigate the expressions and clinical significance of CLEC4D and CARD9 in colon tissue in patients with inflammatory bowel disease(IBD).Methods:A total of 48 IBD patients from October 2016 to June 2018 at the Second Affiliated Hospital of Zhengzhou University were enrolled,including 36 patients with ulcerative colitis(UC)and 12 patients with Crohn’s disease(CD).And 30 adjacent normal colon tissues in patients with colon cancer were served as controls.Immunohistochemistry was used to detect the expressions of CLEC4D and CARD9 in colon tissue.And their correlations with clinical characteristics were analyzed.Results:The expressions of CLEC4D and CARD9 in UC group and CD group were significantly higher than those in control group(P<0.05),however,no significant differences in expressions of CLEC4D and CARD9 were found between UC group and CD group(P>0.05).The expressions of CLEC4D and CARD9 in UC group were positively correlated with Mayo score and Baron grade(P<0.05).The expressions of CLEC4D and CARD9 in CD group were positively correlated with CDAI score(P<0.05),but not with SES-CD score(P>0.05).The expressions of CLEC4D and CARD9 in IBD patients were positively correlated with CRP(P<0.05),but not with ESR(P>0.05).Conclusions:The expressions of CLEC4D and CARD9 are elevated in patients with IBD and are closely related to patients’condition.CLEC4D may participate in the intestinal immunity of IBD through the CARD9-related pathway.
