摘要 背景:基质金属蛋白酶-7(MMP-7)是MMPs家族中的一员,可表达于多种器官受损的黏膜上皮细胞中。多配体蛋白聚糖-1(SDC-1)是MMP-7的作用底物之一,在促进组织修复、维持肠屏障功能等方面发挥重要作用。目的:探讨MMP-7和SDC-1在炎症性肠病(IBD)中的表达及其意义。方法:收集郑州大学第二附属医院2017年1月—2018年1月住院活动期IBD患者的肠组织标本45例和外周血标本50例,同期非IBD肠组织标本30例和外周血标本35例作为对照组。采用免疫组化法检测肠组织MMP-7、SDC-1表达,ELISA法检测血清SDC-1含量。结果:MMP-7在IBD患者的肠组织中呈高表达,特别是溃疡边缘的肠上皮细胞。与对照组相比,IBD患者肠组织MMP-7表达增高、SDC-1表达降低,血清SDC-1含量增高,差异均有统计学意义(P <0. 05)。Spearman相关系数分析显示,肠组织MMP-7表达与IBD疾病活动度呈显著正相关(溃疡性结肠炎:rs=0. 519,P <0. 05;克罗恩病:rs=0. 583,P <0. 05),与肠组织SDC-1表达呈显著负相关(rs=-0. 646,P <0. 05)。结论:MMP-7在IBD患者肠组织中表达增高,通过剪切SDC-1影响受损肠组织修复,参与IBD的发生、发展。 Background:Matrix metalloproteinase-7(MMP-7),a member of the MMPs family,is expressed in injured mucosal epithelial cells of many organs.Syndecan-1(SDC-1),a substrate of MMP-7,plays an important role in tissue repair and maintaining intestinal barrier function.Aims:To investigate the expressions and significance of MMP-7 and SDC-1 in inflammatory bowel disease(IBD).Methods:Forty-five intestinal mucosa samples and fifty peripheral blood samples from inpatients with active IBD were collected during Jan.2017 to Jan.2018 at the Second Affiliated Hospital of Zhengzhou University.Thirty intestinal mucosa samples and thirty-five peripheral blood samples from non-IBD subjects were served as controls.Expressions of MMP-7 and SDC-1 in intestinal mucosal tissue were detected by immunohistochemistry,and serum SDC-1 level was determined by ELISA.Results:MMP-7 was highly expressed in intestinal mucosal tissue of IBD patients,especially in mucosal epithelium bordering the ulcer area.Compared with control group,the expression of MMP-7 was significantly increased and that of SDC-1 was significantly reduced in intestinal mucosal tissue of IBD patients(P<0.05).In contrast,the serum level of SDC-1 was significantly increased in IBD patients(P<0.05).Spearman correlation coefficient analysis revealed a positive correlation between MMP-7 expression and the disease activity of IBD(r s=0.519,P<0.05 for ulcerative colitis;r s=0.583,P<0.05 for Crohn’s disease),and a negative correlation between MMP-7 and SDC-1 in inflamed mucosa(r s=-0.646,P<0.05).Conclusions:Increased MMP-7 expression in intestinal mucosal tissue may contribute to the initiation and development of IBD via cleaving SDC-1 and impairing mucosal healing.
