后路椎弓根螺钉固定术联合融合治疗对屈曲牵张性损伤的临床疗效

摘要 目的观察后路椎弓根螺钉固定术联合融合治疗对屈曲牵张性损伤的临床疗效。方法将我院收治的80例屈曲牵张性损伤患者根据手术方式的不同分为对照组和观察组,各40例。对照组采用开放性椎弓根螺钉内固定术联合融合治疗,观察组采用经皮后路椎弓根螺钉固定术联合融合治疗。比较两组的治疗效果。结果观察组的脊柱功能恢复优良率高于对照组(P<0.05);术后1个月,两组VAS评分均降低,且观察组低于对照组(P<0.05)。术后6个月,两组Cobb角均减小,椎体前缘高度均增加,且观察组优于对照组(P<0.05)。两组并发症总发生率无显著差异(P>0.05)。结论经皮后路椎弓根螺钉固定术联合融合术治疗屈曲牵张性损伤安全有效,可明显减轻患者疼痛、促进患者脊椎功能及结构形态的恢复,且术后并发症少。 Objective To observe the clinical effect of posterior pedicle screw fixation combined with fusion therapy on flexion-distraction injury. Methods Eighty patients with flexion-distraction injury in our hospital were divided into control group and observation group according to different surgical methods, with 40 cases in each group. The control group was treated with open pedicle screw fixation combined with fusion therapy, while the observation group was treated with percutaneous posterior pedicle screw fixation combined with fusion therapy. The therapeutic effects of the two groups were compared. Results The excellent and good rate of spinal function recovery in the observation group was higher than that in the control group(P <0.05). At one month after operation, the VAS scores of both groups decreased, and that in the observation group was lower than the control group(P<0.05). At six months after operation, the Cobb angle of both groups decreased, the height of the anterior vertebral body increased, and those in the observation group were better than the control group(P<0.05). There was no significant difference in the total incidence of complications between the two groups(P>0.05). Conclusion Percutaneous posterior pedicle screw fixation combined with fusion therapy is safe and effective in the treatment of flexion-distraction injury, which can obviously relieve the pain of patients, promote the recovery of vertebral function and structure and has fewer complications after operation.
