摘要
目的探讨高频震荡通气联合肺表面活性物质治疗新生儿胎粪吸入综合征的临床效果。方法将我院收治的76例胎粪吸入综合征患儿按照平行对照法分为研究组(38例,高频震荡通气+肺表面活性物质)与对照组(38例,常频机械通气+肺表面活性物质)。比较两组的治疗效果。结果 N1、N2、N3、N4时,两组PaO2、pH、PaO2/FiO2高于N0时,PaCO2、FiO2低于N0时,且研究组优于对照组(P<0.05)。两组N0、N1、N2、N3、N4的HR、MAP比较无显著差异(P>0.05)。研究组呼吸机使用时间、住院时间均短于对照组(P<0.05)。两组治疗总有效率无显著差异(P>0.05)。结论高频震荡通气联合肺表面活性物质治疗新生儿胎粪吸入综合征效果显著,可改善患儿血气分析指标及氧合指标,促使患儿早日康复。
Objective To investigate the clinical effect of high frequency oscillatory ventilation combined with pulmonary surfactant in the treatment of neonatal meconium aspiration syndrome. Methods Seventy-six children with meconium aspiration syndrome in our hospital were divided into study group(38 cases, high frequency oscillatory ventilation+pulmonary surfactant) and control group(38 cases, normal frequency mechanical ventilation +pulmonary surfactant) according to parallel control method. The therapeutic effects of the two groups were compared. Results At N1, N2, N3 and N4, the PaO2,pH and PaO2/Fi O2 of the two groups were higher than those at N0, while PaCO2 and Fi O2 were lower than those at N0, and those in the study group were better than the control group(P<0.05). There were no significant differences in HR and MAP between the two groups at N0, N1, N2, N3 and N4(P>0.05). The ventilator use time and hospitalization time in the study group were shorter than those in the control group(P<0.05). There was no significant difference in the total effective rate of treatment between the two groups(P>0.05). Conclusion High frequency oscillatory ventilation combined with pulmonary surfactant in the treatment of neonatal meconium aspiration syndrome has a significant effect, which can improve the blood gas analysis indexes and oxygenation indexes of children and promote the early recovery of children.
