摘要
目的探索治未病思想在Rh(D)阴性血型孕妇自体输血中的应用价值。方法选取32例Rh(D)阴性血型孕妇在临产前1~3周实施自体输血的血液采集,血液置于2~6℃贮血冰箱内保存备用。其中14例Rh(D)阴性血型孕妇产后回输自体血液。比较采血前、后及回输前、后血液指标水平、妊娠结局情况。结果 32例Rh(D)阴性血型孕妇采血前、后的RBC、Hb、PLT、Hct、NST比较,差异均无统计学意义(P>0.05);14例孕妇血液回输前、后的RBC、Hb、PLT、Hct比较,差异均无统计学意义(P>0.05);阴道分娩20例,剖宫产12例;新生儿Apgar评分均为8~10分。结论"未病先防""既病防变"的治未病思想应用于临床Rh(D)阴性血型孕妇妊娠晚期贮存式自体输血中,不仅可避免输血相关疾病风险,又可缓解稀缺的血型资源,保证分娩安全,值得临床推广应用。
Objective To explore the application value of preventive treatment of disease method on autotransfusion of Rh(D) negative blood type pregnant women. Methods Thirty-two cases pregnant women with Rh(D) negative blood type were selected collect blood from autotransfusion 1-3 weeks before parturition. The blood was stored in a refrigerator at2-6 ℃ for reserve. Among them, 14 cases of Rh(D) negative pregnant women received autologous blood transfusion after delivery. The blood indexes before and after blood collection and before and after transfusion and pregnancy outcome were compared. Results There were no significant differences in RBC, Hb, PLT, Hct and NST of 32 cases of Rh(D) negative pregnant women before and after blood collection(P>0.05). There were no significant differences in RBC, Hb, PLT and Hct indexes before and after blood transfusion in 14 pregnant women(P>0.05). A total of 20 cases were vaginal delivery, 12 cases were cesarean section, and neonatal Apgar score was 8-10. Conclusion The ideas of "prevention before disease" and "prevention of disease and change" in the storage autotransfusion of late pregnancy for Rh(D) negative pregnant women can not only avoid the risk of blood transfusion-related diseases, but also alleviate the scarce blood group resources and ensure the safety of delivery. It is worthy of popularization and application in clinic.
