手术治疗甲状腺良恶性结节的效果

Effect of surgical treatment of benign and malignant thyroid nodules

目的探讨手术治疗甲状腺良恶性结节的效果,为本病临床诊治提供参考依据。方法选择我科收治的75例甲状腺结节患者为研究对象,依据病理诊断结果将其分为良性结节组(38例)和恶性结节组(37例),两组均根据患者病情制定个体化手术方案,比较两组疗效、基本手术指标、并发症发生情况及复发率。结果两组患者手术成功率及治疗总有效率均为100.0%。良性结节组手术时间与术中出血量均优于恶性结节组(P<0.05);两组术后住院时间比较,差异无统计学意义(P>0.05)。术后随访,恶性结节组甲状腺功能低下与低钙血症发生率高于良性结节组(P<0.05);两组甲状旁腺功能减退与声音嘶哑发生率无显著差异(P>0.05)。结论手术治疗甲状腺良恶性结节效果确切,复发率低,临床可根据患者实际酌情选择手术方案。

Objective To investigate the effect of surgical treatment of benign and malignant thyroid nodules, and to provide reference for clinical diagnosis and treatment of this disease. Methods Seventy-five patients with thyroid nodules admitted in our department were selected as the study objects. They were divided into benign nodule group (38 cases) and malignant nodule group (37 cases) according to results of pathological diagnosis. Individualized surgical procedures were formulated according to the patients' condition in both groups. The curative effects, basic surgical indicators, the incidences of complications and recurrence rates of the two groups were compared. Results The success rates of operation and the total effective rates of treatment were 100.0% in both groups. The operation time and intraoperative bleeding volume in the benign nodule group were better than those in the malignant nodule group (P<0.05), and there was no significant difference in postoperative hospitalization time between the two groups (P>0.05). The incidences of hypothyroidism and hypocalcemia in the malignant nodule group were higher than those in the benign nodule group (P<0.05), and there were no significant differences in hypoparathyroidism and hoarseness between the two groups (P>0.05). Conclusion Surgical treatment of benign and malignant thyroid nodules has a definite effect and a low recurrence rate. Surgical options can be selected according to the actual situation of patients in clinic.
