针刺疗法结合中药灌肠联合西医常规治疗ICU重症感染并胃肠功能障碍的效果评价

Evaluation on curative effects of acupuncture therapy and herbal enema combined with routine western medicine in treatment of severe infection and gastrointestinal dysfunction in ICU

目的探讨针刺疗法结合中药灌肠联合西医常规治疗ICU重症感染并胃肠功能障碍的疗效。方法选取2016年3月～2018年6月我院的ICU重症感染并胃肠功能障碍的患者60例,用随机数字表法分为对照组(29例)和实验组(31例)。对照组给予常规对症治疗,在此基础上,实验组给予针刺疗法联合中药灌肠治疗,比较两组患者的肠鸣音恢复时间、开始排气时间、开始排便时间、自觉腹胀缓解时间、胃肠功能评分、急性生理与慢性健康Ⅱ评分(APACHE-Ⅱ)、降钙素原(PCT)、C反应蛋白(CRP)、肿瘤坏死因子(TNF-α)和白介素-6(IL-6)。结果实验组患者的肠鸣音恢复时间、开始排气时间、开始排便时间、自觉腹胀缓解时间均显著短于对照组,差异有统计学意义(P <0.05);治疗后实验组的胃肠功能评分和APACHE-Ⅱ均显著低于对照组,差异有统计学意义(P <0.05);治疗后实验组的PCT、CRP、TNF-α和IL-6显著低于对照组(P <0.05)。结论在治疗ICU重症感染并胃肠功能障碍患者中应用针刺疗法结合中药灌肠联合西医常规治疗的治疗效果显著,能促进患者胃肠功能和感染的恢复。

Objective To explore curative effects of acupuncture therapy and herbal enema combined with routine western medicine in treatment of severe infection and gastrointestinal dysfunction in ICU. Methods 60 patients with severe infection and gastrointestinal dysfunction in ICU in our hospital from March 2016 to June 2018 were selected.According to the random number table method,they were divided into the control group(29 cases) and the experimental group(31 cases).The control group was given routine symptomatic treatment while the experimental group was given acupuncture therapy and herbal enema on basis of the control group.The recovery time of intestinal sound,time for starting exhaust,time for starting defecation,remission time of conscious abdominal distention,gastrointestinal function score,acute physiology and chronic health Ⅱ score(APACHE-Ⅱ),procalitonin(PCT),C-reactive protein(CRP),tumor necrosis factor-α(TNF-α) and interleukin-6(IL-6) of patients were compared between the two groups. Results The recovery time of intestinal sound, time for starting exhaust,time for starting defecation and remission time of conscious abdominal distention of patients in the experimental group were all shorter than those in the control group.The differences were statistically significant(P < 0.05).The gastrointestinal function score and APACHE-Ⅱ score after treatment of the experimental group were both lower than those of the control group.The differences were statistically significant(P < 0.05).After treatment,PCT,CRP,TNF-α and IL-6 in the experimental group were significantly lower than those in the control group(P < 0.05). Conclusion Acupuncture therapy and herbal enema combined with routine western medicine in treatment of patients with severe infection and gastrointestinal dysfunction in ICU has significant curative effects,which can promote the recovery of gastrointestinal function and infection of patients.
