摘要
目的探讨磁共振引导下聚焦超声刀(HIFU)治疗子宫肌瘤的术后评价。方法选取2017年6月~2018年1月本院收治的子宫肌瘤患者151例,依据随机数字表法分为HIFU组(n=75例)和腹腔镜组(n=76例),腹腔镜组给予腹腔镜下剔除术治疗,HIFU组给予磁共振引导下HIFU治疗,比较两组术后疗效。结果 HIFU组手术时间、术中出血量、住院时间明显低于腹腔镜组,HIFU组手术费用明显高于腹腔镜组,差异有统计学意义(P <0.05);HIFU组术后并发症发生率明显低于腹腔镜组,差异有统计学意义(P <0.05);HIFU组和腹腔镜组术后6个月复发率比较,差异无统计学意义(P> 0.05)。结论磁共振引导下HIFU治疗子宫肌瘤的术后疗效良好,且具有创伤小、并发症少、康复快等优点,值得临床作进一步推广。
Objective To explore the postoperative evaluation of magnetic resonance guided focused ultrasound surgery(HIFU) in the treatment of hysteromyoma. Methods 151 patients with hysteromyoma who were admitted and treated in our hospital from June 2017 to January 2018 were selected. According to the random number table method,they were divided into HIFU group(75 cases) and laparoscopic group(76 cases).Laparoscopy group was treated by laparoscopic removal while HIFU group was treated with HIFU guided by MRI.The postoperative outcomes of the two groups were compared. Results The operative time,intraoperative blood loss and hospital stay in HIFU group were significantly lower than those in laparoscopic group,and surgery cost of HIFU group was significantly higher than that in laparoscopy group.The differences were statistically significant(P < 0.05).The incidence of postoperative complications in HIFU group was significantly lower than that in laparoscopic group, and the difference was statistically significant(P < 0.05).There was no significant difference in the recurrence rate between the HIFU group and the laparoscopic group on 6 months after operation(P > 0.05). Conclusion MRI-guided HIFU in the treatment of hysteromyoma has good postoperative curative effects.It has advantages of less trauma,fewer complications and faster recovery,which is worthy of further clinical promotion.
