Skp2、P27和CyclinE在宫颈癌组织中的表达及与临床病理特征的关系 被引量:8

Expression of Skp2、P27 and CyclinE protein and their correlations with clinicopathological features of invasive cervical carcinoma

摘要 目的探讨浸润性宫颈癌(ICC)组织中Skp2、P27和CyclinE的表达情况及其临床病理意义。方法选取2012年1月~2018年1月我院病理科存档石蜡包埋ICC组织标本70例为ICC组,宫颈上皮内瘤变(CIN)组织标本30例为CIN组和正常子宫颈鳞状上皮(NCE)组织标本50例为NCE组。采用免疫组织化学法检测各组标本中Skp2、P27和CyclinE表达情况。结果 ICC组、CIN组、NCE组Skp2、CyclinE蛋白表达的阳性呈明显降低趋势,P27蛋白表达的阳性呈明显升高趋势,差异有统计学意义(P <0.05)。在ICC组中,Skp2蛋白、CyclinE蛋白及P27蛋白表达与年龄、肿瘤大小、临床分期无明显关系(P> 0.05);而与组织学Ⅲ级、间质浸润>浅肌层、盆腔淋巴结转移阳性和脉管浸润阳性者Skp2蛋白和CyclinE蛋白表达阳性率分别显著高于组织学≤Ⅱ级、间质浸润≤浅肌层、盆腔淋巴结转移阴性和脉管浸润阴性者,P27蛋白表达阳性率分别显著低于组织学≤Ⅱ级、间质浸润≤浅肌层、盆腔淋巴结转移阴性和脉管浸润阴性者,差异有统计学意义(P <0.05)。随访资料显示,ICC组生存期≥5年者38例,生存期≥5年者Skp2和CyclinE蛋白表达阳性率明显低于生存期<5年者,P27蛋白表达阳性率明显高于生存期<5年者,差异有统计学意义(P <0.05)。结论 Skp2、P27和CyclinE共同参与了宫颈癌的发生发展。Skp2和CyclinE蛋白高表达可能是宫颈组织恶变的重要生物学标志,Skp2和CyclinE过度表达,伴P27的低表达提示肿瘤具有更高的侵袭性及不良预后。 Objective To investigate the expression of Skp2,P27 and CyclinE in invasive cervical cancer(ICC) and its clinicopathological significance. Methods Seventy paraffin-embedded ICC tissue specimens from January 2012 to January 2018 in pathology department of our hospital were selected as ICC group,30 cervical intraepithelial neoplasia(CIN) tissue specimens as CIN group and 50 normal cervical squamous epithelium(NCE) tissue specimens as NCE group.The expression of Skp2,P27 and CyclinE was detected by immunohistochemistry. Results The expression of Skp2 and CyclinE protein in ICC group,CIN group and NCE group decreased significantly,while the expression of P27 protein increased significantly(P < 0.05).In ICC group,the expression of Skp2 protein,CyclinE protein and P27 protein was not significantly correlated with age,tumor size and clinical stage(P > 0.05),while the expression of Skp2 protein and CyclinE protein in patients with histological grade Ⅲ,interstitial infiltration > superficial muscle layer,positive pelvic lymph node metastasis and positive vascular infiltration were significantly higher than those in patients with histological grade ≤ II,interstitial infiltration ≤ superficial muscle layer and pelvic lymph node infiltration,respectively.The positive expression rate of P27 in pelvic lymph node metastasis,vascular infiltration and interstitial infiltration(> superficial muscular layer) was significantly lower in patients with negative nodal metastasis and vascular infiltration(P < 0.05).Follow-up data showed that 38 patients in ICC group survived for more than 5 years.The positive rates of Skp2 and CyclinE protein expression in ICC group survived for more than 5 years were significantly lower than those in ICC group survived for less than 5 years,and the positive rates of P27 protein expression were significantly higher than those in ICC group survived for less than 5 years.The difference was statistically significant(P < 0.05). Conclusion Skp2,P27 and CyclinE are involved in the occurrence and development of cervical cancer.High expression of Skp2 and CyclinE may be an important biological marker of cervical malignancy.Overexpression of Skp2 and CyclinE and low expression of P27 suggest that tumors have higher invasiveness and poor prognosis.
