腹腔镜与开腹胆囊切除术对急性结石性胆囊炎患者胃肠功能的影响

Effect of laparoscopic and open cholecystectomy on gastrointestinal function of patients with acute calculous cholecystitis

目的观察腹腔镜与开腹胆囊切除术对急性结石性胆囊炎患者胃肠功能的影响。方法随机选取我院2017年5月～2018年5月接受的64例急性结石性胆囊炎患者为本次研究对象,在征得患者家属、患者本人同意的情况下按照术式不同将所有患者分为对照组(32例:开腹胆囊切除术)与实验组(32例:腹腔镜胆囊切除术),比较两组患者预后情况。结果实验组患者术后肠鸣音恢复耗时、肛门首次排气时间、肛门首次排便时间均少于对照组,P <0.05。实验组患者术后24h、术后48h血清CRP均明显优于对照组,P<0.05。实验组术后胃肠道功能Ⅰ级患者明显多于对照组(62.50% VS 53.13%),P <0.05。实验组患者术后不良反应发生率为9.38%,明显低于对照组,P <0.05。结论对于急性结石性胆囊炎患者选择予行腹腔镜胆囊切除术有利于患者术后胃肠功能的恢复。

Objective To observe the effect of laparoscopic and open cholecystectomy on gastrointestinal function of patients with acute calculous cholecystitis. Methods 64 patients with acute calculous cholecystitis who were admitted to our hospital from May 2017 to May 2018 were randomly selected as research objects.With the consent of the patient’s family members and the patient himself,they were divided into control group(32 cases,open cholecystectomy) and experimental group(32 cases,laparoscopic cholecystectomy) according to different operation methods.The prognosis of the two groups was compared. Results The recovery time of bowel sounds,the time of first anal exhaust and the time of first anal defecation in the experimental group were shorter than those in the control group(P < 0.05).The serum CRP of 24 hours and 48 hours after operation in the experimental group was significantly better than that in the control group,P < 0.05.Patients with grade Ⅰ postoperative gastrointestinal function in the experimental group were significantly more than those in the control group(62.50% VS 53.13%),P < 0.05.The incidence of postoperative adverse reactions in the experimental group was 9.38%,significantly lower than that in the control group,P < 0.05. Conclusion Laparoscopic cholecystectomy for patients with acute calculous cholecystitis is beneficial to the postoperative recovery of gastrointestinal function.
