血液肿瘤住院患儿营养风险筛查及影响因素研究

Study on the nutrition risk screening and effect factors for malnutrition in hospitalized children with hematological tumor

目的调查血液肿瘤住院患儿营养风险及营养不良发生状况,并分析其影响因素,为临床开展营养风险初筛项目、合理评估营养状况提供依据。方法收集血液肿瘤住院患儿350例的临床资料,应用中文版儿童营养不良风险评分表(Chinese Version Paediatric Yorkhill Malnutrition Score,CVPYMS)筛查血液肿瘤住院患儿营养风险及营养不良发生状况,并分析其影响因素。结果应用CVPYMS,350例住院患儿中存在高度营养风险69例(19.72%),中度营养风险249例(71.14%),低度营养风险32例(9.14%)。根据中国儿科肠内肠外营养支持临床应用指南营养养不良分级标准,营养不良检出率是5.71%,其中中度营养不良18例(5.14%),重度营养不良2例(0.57%)。Logistic回归分析结果显示,年龄、化疗次数、肿瘤分期是血液肿瘤住院患儿发生营养不良风险的影响因素。结论血液肿瘤住院患儿营养不良发生率高,年龄、化疗次数、肿瘤分期是对患儿营养风险程度产生影响的关键因素。

Objective To investigate the nutritional risk and malnutrition of hospitalized children with blood tumor and analyze its effect factors,providing evidence for the clinical nutritional risk screening program and the reasonable assessment of nutritional status.Methods Based on the collected data of 350 hospitalized children with blood tumor,Chinese Version Paediatric Yorkhill Malnutrition Score(CVPYMS)was employed to analyze the present situation and effect factors of nutritional risk and malnutrition in hospitalized children with blood tumor.Results 69(19.72%)of the 350 hospitalized children's nutritional risk was high,249(71.14%)was moderateand 32(9.14%)was low.According to the malnutrition classification criteria in Chinese Guidelines for Clinical Application of Paediatric Parenteral Nutrition Support,the malnutritiondetection rate was 5.71%,among which 18(5.14%)were moderately malnourished,and 2(0.57%)were severe malnutrition.Logistic regression analysis indicated that age,chemotherapy period and tumor stage were the independent risk factors affecting themalnutrition risk among hospitalized children with blood tumor.Conclusion The incidence of malnutrition among hospitalized children with blood tumor is high;age,chemotherapy period and tumor stage are the critical factors affecting themalnutrition risk among hospitalized children with blood tumor.
