老年突发性聋患者细胞因子的变化特征研究

Characteristics of Cytokines in the Elderly with Sudden Sensorineural Hearing Loss

目的观察老年突发性聋(sudden sensorineural hearing loss,SSNHL)患者血清中TGF-β1、TNF-α及IL-1β的表达及意义,为SSNHL的诊治提供依据。方法选择2016年10月—2018年9月老年SSNHL患者40例设为老年SSNHL组,将30例年龄和性别匹配的老年非SSNHL患者设为对照组(老年非SSNHL对照组)及34例中青年SSNHL患者设为对照组(中青年SSNHL对照组)。采用ELISA法分别检测各组血清细胞因子水平,并进行统计学分析与比较。同时对老年SSNHL分型血清中细胞因子水平进行评估和比较。根据治疗效果将老年SSNHL组分为有效亚组和无效亚组,对2亚组血清细胞因子水平进行比较。结果老年SSNHL组血清TGF-β1水平明显低于老年非SSNHL对照组(P<0.05),而与中青年SSNHL对照组相比的差异无统计学意义(P>0.05);老年SSNHL组血清TNF-α和IL-1β表达水平与2个对照组相比的差异无统计学意义(P>0.05)。血清TGF-β1、TNF-α和IL-1β的表达水平在老年SSNHL各型间的差异无统计学意义(P>0.05)。2个老年SSNHL亚组TGF-β1、TNF-α和IL-1β水平的差异均无统计学意义(P>0.05)。表明TGF-β1、TNF-α和IL-1β水平与老年SSNHL分型和治疗结果无明显关系。结论 TGF-β1在老年SSNHL的发生中可能有重要作用,可为老年SSNHL的诊治提供参考。

Objective To observe the expressions of TGF-β1,TNF-αand IL-1βin the serum of the elderly with sudden sensorineural hearing loss(SSNHL)and the significance as as to provide guidance for the diagnosis and treatment.Methods During the period from Oct.,2016 to Sept.,2018,40 elderly with SSNHL were selected to be SSNHL group while 30 elderly of matched age and sex with no SSNHL were selected to be elderly control group and 34 cases of young and middle age with SSNHL were selected to be young control group;ELISA was applied in detecting the levels of cytokines of the cases in each group and statistical analysis and comparison were performed;the levels of cytokines in the serum of the elderly with SSNHL of different subtypes were evaluated;the SSNHL group was further divided into 2 subgroups:effective subgroup and ineffective subgroup according to the curative effect,and the serum cytokine levels of cases in the 2 subgroups were compared.Results The level of TGF-β1 of the cases in SSNHL group was much lower than that in elderly control group(P<0.05)while it was of no statistical difference from that in young control group(P>0.05);the expression levels of TNF-αand IL-1βin SSNHL group were of no statistical difference from those in the 2 control groups(P>0.05);no statistical differences were found in the expression levels of TGF-β1,TNF-αand IL-1βamong the subtypes in SSNHL group(P>0.05);there existed no statistical differences in the expression levels of TGF-β1,TNF-αand IL-1βbetween effective subgroup and ineffective subgroup(P>0.05);the levels of TGF-β1,TNF-αand IL-1βwere of no obvious correlation to subtypes and the curative outcome in the elderly.Conclusions TGF-β1 plays an important role in the occurrence of SSNHL in the elderly,which may be a reference in the diagnosis and treatment.
