医院-社区协同健康管理模式对脑卒中恢复期人群二级预防的效果研究

Effect of hospital-community collaborative health management model on secondary prevention of stroke patients during recovery

目的探讨医院-社区协同健康管理模式对脑卒中恢复期人群二级预防的效果。方法选择2017年1月至2018年3月70例脑卒中恢复期患者作为研究对象,随机等分为干预组和对照组。对照组实施常规出院健康教育,干预组在此基础上给予医院-社区协同健康管理。比较两组出院时及出院6个月时患者脑卒中防治知识、健康促进生活方式、生活质量评分结果。结果干预组出院6个月时脑卒中防治知识及健康促进生活方式评分均高于出院时及对照组出院6个月时评分,差异均有统计学意义(P <0. 05)。出院6个月时两组患者生活质量均得到改善,但干预组生活质量各维度得分高于对照组,差异有统计学意义(P <0. 05)。结论医院-社区协同健康管理模式能提高脑卒中恢复期患者脑卒中防治知识、健康促进生活方式及生活质量,达到较好的二级预防效果。

Objective To discuss the effect of hospital-community collaborative health management model on secondary prevention of stroke patients during recovery. Methods To select 70 cases of patients with cerebral apoplexy during recovery period from January 2017 to March 2018 as the study subjects. They were divided into intervention group and control group with the same number of patients randomly. The control group was given routine discharge health education, while the intervention group was given hospital-community collaborative health management on this basis. The results of stroke prevention knowledge, health promotion lifestyle and quality of life scores were compared between the two groups at discharge and at 6 months after discharge. Results The stroke prevention knowledge and health promotion lifestyle scores of the intervention group at 6 months after discharge were higher than those in the control group at discharge and 6 months after discharge,and the difference was statistically significant(P<0.05). The quality of life of both groups was improved at 6 months after discharge, but the scores of qualities of life in the intervention group were higher than those in the control group,and the difference was statistically significant(P<0.05). Conclusion The hospital-community collaborative health management model can improve stroke prevention knowledge, health promotion lifestyle and quality of life in patients with stroke during recovery period. Therefore, it can achieve better secondary prevention effect.
