舒适护理在胆管结石患者行ERCP+EST治疗中的应用

Application of comfort nursing in treatment of ERCP combined with EST for patients with bile duct stones

目的探讨舒适护理在胆管结石患者行内镜下逆行胰胆管造影术(ERCP)+十二指肠乳头切开取石术(EST)围术期护理中的应用效果。方法选取2017年2月至2018年2月我院收治的行ERCP+EST诊断治疗的胆管结石患者96例作为研究对象,按照就诊顺序编号,采用数字随机表法将其等分为对照组和观察组。对照组采用常规护理,观察组在对照组基础上采用舒适护理。比较两组患者疼痛评分(VAS)、皮肤舒适度评分、术后并发症发生率。结果术前两组患者VAS与皮肤舒适度评分比较差异无统计学意义(P> 0. 05);术后3 d,观察组患者VAS评分、术后并发症发生率明显低于对照组(P <0. 05),皮肤舒适度评分明显高于对照组(P <0. 05)。结论舒适护理可提高胆管结石患者行ERCP+EST治疗的皮肤舒适度,减轻疼痛程度,降低术后并发症发生率,值得推广应用。

Objective To explore the application effect of comfort nursing in perioperative nursing of patients with bile duct stones who received endoscopic retrograde cholangiopancreatography(ERCP) combined with endoscopic sphincterotomy(EST). Methods Research subjects are 96 patients who were diagnosed with bile duct stones and treated by ERCP+EST and who were enrolled in our hospital from February 2017 to February 2018. Based on the treatment order, the patients were equally divided into the control group and the study group using table of random numbers. Routine care is applied to the control group and the same plus comfort nursing on the study group. Contrasts were made between two groups in terms of patients visual analogue scale(VAS), skin comfort score and postoperative complication rate. Results There was no significant difference in VAS and skin comfort score between the two groups before operation(P>0.05).At 3 days after operation, the VAS score and postoperative complication rate of the study group were significantly lower than those of the control group(P<0.05), and the skin comfort score was significantly higher than that of the control group(P<0.05). Conclusion Comfort care can improve the skin comfort of patients with bile duct stones who are treated with ERCP+EST, reduce their pain and incidence rate of postoperative complications,which was worth popularizing and applying.
