卫生服务整合视角下医养结合模式的案例研究——基于福建省漳州市某医院和养护院

A Case Study on the Model of Integrated Care for the Elderly:Based on a Hospital and a Nursing Home in Zhangzhou,Fujian

分析案例开展医养结合服务的经验教训,探索医养结合服务开展的可能路径。基于卫生服务整合视角,从系统整合、组织整合、专业整合、服务整合4个层面梳理案例开展医养结合的措施及面临的问题。措施包括出台整体规划和建立多部门协调机制;所有权一致和位置毗邻;构建医养服务团队和开展人员激励;开展全面综合和连续协调的服务。提出基于老年人需求评估开展服务整合,通过专业和组织整合调配医养资源,系统整合全程规范管理的医养结合服务探索路径。

To analyze how the two organizations provide integrated care for the elderly,and to explore a possible path to develop the integrated care for the elderly.The framework for integrated care,which is composed of the elements of system integration,organizational integration,professional integration and clinical integration,was used to analyze their measures and problems.Measures include the introduction of overall planning and the establishment of a multi-sectoral coordination mechanism;coherence of ownership and adjacent location;the combination of service teams and staff incentives;and the development of comprehensive and continuous coordination of services.The ser- vice integration should be carried out based on the needs assessment of the elderly,the resources should be allocated through professional and organizational integration,and the care services for the elderly should be systematically integrated in a standardized way.
