无创呼吸机在重症哮喘治疗中的应用及肺功能指标评价 被引量:1

摘要 目的探讨无创呼吸机在重症哮喘治疗中的应用及肺功能。方法方便选择2015年3月—2018年4月该院收治100例重症哮喘患者。按照治疗分组,对照组采取常规内科药物治疗,研究组则采取常规内科药物联合无创呼吸机治疗。比较两组疗效;胸闷、呼吸困难等重症哮喘症状缓解时间;治疗前后患者肺功能FEV1、PEF、血气指标血氧饱和度、酸碱度、血氧分压和二氧化碳分压;不良反应。结果研究组疗效49例(98.00%)较高(χ~2=9.043,P<0.05)。研究组胸闷、呼吸困难等重症哮喘症状缓解时间更好(P<0.05),研究组胸闷、呼吸困难、其他症状缓解时间分别是(5.56±1.42)、(5.11±1.42)、(5.13±0.24)d,对照组胸闷、呼吸困难、其他症状缓解时间分别是(7.42±2.25)、(7.42±2.21)、(7.23±1.21)d(t=5.911、6.821、5.814)。治疗后研究组FEV1、PEF、血氧饱和度、PaO2、PaCO2、pH分别是(2.01±0.32)L、(4.24±0.62) L/s、(81.14±2.55)%、(94.74±3.55)mmHg、(42.14±7.55) mmHg、(7.38±0.02),对照组分别是(1.53±0.23) L、(3.41±0.32) L/s、(96.83±1.22)%、(56.23±2.74) mmHg、(78.83±8.92) mmHg、(7.30±0.02)(t=5.911、6.821、5.814、6.942、7.014、5.711,P<0.05)。研究组不良反应和对照组接近(χ~2=0.000,P>0.05)。结论常规内科药物联合无创呼吸机治疗重症哮喘的疗效好,可更好改善肺功能FEV1、PEF、血气指标血氧饱和度、酸碱度、血氧分压和二氧化碳分压。 Objective To investigate the application of non-invasive ventilator in the treatment of severe asthma and pulmonary function. Methods 100 patients with severe asthma from March 2015 to April 2018 were convenient selected and enrolled in our hospital. According to the treatment group, the control group received conventional medical treatment, and the study group received conventional medical treatment combined with non-invasive ventilator. Comparison of the two groups of efficacy;chest tightness, dyspnea and other symptoms of severe asthma relief;before and after treatment, lung function FEV1, PEF, blood gas parameters oxygen saturation, pH, blood oxygen partial pressure and carbon dioxide partial pressure;adverse reactions. Results The study group had a higher efficacy of 49 cases(98.00%)(χ^2=9.043, P<0.05). The study group had better relief time for severe asthma symptoms such as chest tightness and dyspnea(P <0.05). The study group had chest tightness, dyspnea, and other symptom relief times(5.56±1.42),(5.11±1.42), and(5.13±0.24) d, the control group chest tightness, dyspnea, and other symptom relief time were(7.42±2.25),(7.42±2.21),(7.23±1.21) d(t=5.911, 6.821,5.814). The adverse reactions of the study group were close to those of the control group(χ^2=0.000, P>0.05). Conclusion Conventional medical drugs combined with non-invasive ventilator are effective in the treatment of severe asthma,which can improve lung function FEV1, PEF, blood gas oxygen saturation, p H, blood oxygen partial pressure and carbon dioxide partial pressure.
