腹腔镜联合胆道镜治疗胆管结石的手术配合及护理

Surgical Cooperation and Nursing of Laparoscopic Combined with Choledochoscopy for Bile Duct Stones

目的分析腹腔镜联合胆道镜治疗胆管结石患者的手术配合要点,总结有效的护理措施。方法方便选取该院2017年1月—2018年8月期间收治的120例胆管结石患者,所有患者均采用腹腔镜联合胆道镜治疗,根据护理方式的差异将120例患者随机分为实验组(60例,常规护理同时加强手术配合及护理)和对照组(60例,实施常规护理措施),对比两组患者术中出血量、手术时间、胃肠功能恢复时间、住院时间以及术后并发症发生率等情况。结果实验组患者术中出血量明显少于对照组(P<0.05),实验组患者手术时间、胃肠功能恢复时间以及住院时间等指标均明显短于对照组(P<0.05)。实验组患者术后并发症发生率6.7%明显低于对照组25.0%(χ~2=8.835,P<0.05)。结论胆管结石患者采用腹腔镜联合胆道镜治疗过程中加强手术配合及护理更有利于确保手术的顺利开展,降低患者术后并发症发生率,可在临床上广泛推广应用。

Objective To analyze the surgical coordination points of laparoscopic combined with choledochoscopy in the treatment of patients with bile duct stones, and to summarize effective nursing measures. Methods 120 patients with bile duct stones admitted to our hospital from January 2017 to August 2018 were convenient enrolled. All patients were treated with laparoscopic and choledochoscopy. According to the difference of nursing methods, 120 patients were randomly divided into experimental group(60 cases, routine nursing at the same time strengthen surgical coordination and care) and the control group(60 cases, the implementation of routine nursing measures), comparing the intraoperative blood loss, operation time, gastrointestinal function recovery time, hospitalization time and postoperative complication rate and other situation.Results The intraoperative blood loss of the experimental group was significantly lower than that of the control group(P<0.05). The operation time, gastrointestinal function recovery time and hospitalization time of the experimental group were significantly shorter than the control group(P<0.05). The incidence of postoperative complications in the experimental group was 6.7% significantly lower than that in the control group 25.0%(χ^2=8.835, P<0.05). Conclusion The use of laparoscopic combined with choledochoscopy in the treatment of patients with bile duct stones is more conducive to ensuring the smooth operation of the operation and reducing the incidence of postoperative complications. It can be widely applied in clinical practice.
