超声诊断甲状腺占位性病变的临床价值分析 被引量:2

摘要 目的探讨术前超声检查对于甲状腺占位性病变的诊断价值。方法回顾性分析2014年1月—2018年6月间于该院就诊、治疗的手术病理确诊为甲状腺占位性病变64例患者的临床资料,比较术前多普勒彩超检查、手术病理检查关于占位性病变类型及良恶性的诊断结果,总结甲状腺占位性病变在超声中的大小、位置、血流分布等特点,探讨术前超声诊断的临床价值。结果①术前超声诊断结果显示,在64例甲状腺占位性病变患者中有良性病变58例、恶性病变6例,手术病理结果中良性病变56例、恶性病变8例,两种检查方法对于甲状腺占位性病变良恶性的诊断结果比较差异无统计学意义(χ~2=0.080,P=0.777);②在病型方面,超声检查结果提示甲状腺癌占9.38%、甲状腺炎性结节占65.62%、甲状腺囊肿占18.75%、甲状腺腺瘤占6.25%,而手术病理确诊结果中状腺癌占7.81%、甲状腺炎性结节占62.50%、甲状腺囊肿占25.00%、甲状腺腺瘤占4.69%,两种检查方式对于64例甲状腺占位性病变患者疾病病型的诊断结果比较,差异统计学意义(χ~2=0.000、0.034、0.411、0.000,P>0.05)。结论术前多普勒彩超检查能够准确反映甲状腺占位性病灶的位置、数目、结构和周边血流分布情况,并据此对病灶的良恶性及病理类型做出准确判断,有助于提高此类疾病的疗效和预后水平。 Objective To investigate the value of preoperative ultrasonography in the diagnosis of thyroid occupying lesions.Methods A retrospective analysis of the clinical data of 64 patients with thyroid space-occupying lesions diagnosed and treated in our hospital from January 2014 to June 2018,compared with preoperative Doppler color ultrasound examination and surgical pathological examination.The diagnosis of the type of lesions and the diagnosis of benign and malignant,summarize the characteristics of thyroid space-occupying lesions in ultrasound,such as size,location,blood flow distribution,etc.,to explore the clinical value of preoperative ultrasound diagnosis.Results 1.Preoperative ultrasound diagnosis showed that there were 58 benign lesions and 6 malignant lesions in 64 patients with thyroid space-occupying lesions.There were 56 benign lesions and 8 malignant lesions in surgical pathology.There was no significant difference in the diagnosis of benign and malignant thyroid-occupying lesions(χ^2=0.080,P=0.777 5);2.in terms of disease type,the results of ultrasonography showed thyroid cancer accounted for 9.38%,thyroiditis occlusion accounted for 65.62%,the thyroid cyst accounted for 18.75%,and the thyroid adenoma accounted for 6.25%.The pathological diagnosis confirmed that the adenocarcinoma accounted for 7.81%,the thyroid inflammatory nodules accounted for 62.50%,the thyroid cyst accounted for 25.00%,and the thyroid adenoma accounted for 4.69%,the difference between the two examination methods for the diagnosis of 64 patients with thyroid space-occupying disease disease,there is no statistically significant difference(χ^2=0.000,0.034,0.411,0.000,P>0.05).Conclusion Preoperative Doppler ultra sonography can accurately reflect the location,number,structure and peripheral blood flow distribution of thyroid occupying lesions,and make accurate judgments on the benign and malignant lesions and pathological types of the lesions,which will help to improve efficacy and prognosis of such diseases.
