分层护理在血液透析患者临床护理中的应用价值评价

Evaluation of Application Value of Stratified Nursing in Clinical Nursing of Hemodialysis Patients

目的探讨在血液透析患者临床护理中应用分层护理的应用价值。方法选取2016年12月—2018年7月该院接收的55例血液透析患者为基线资料展开研究,按护理措施的不同将其以1:1的比例分为2组,对照组(n=28)和观察组(n=27),给予常规护理的为对照组,实施分层护理干预的为观察组,对比2组患者护理质量评分、生活质量、血液透析导管感染、血栓发生率以及护理满意度。结果观察组基础护理(93.65±3.66)分、专业技能(92.46±1.53)分、护理安全(92.36±4.53)分、病房管理(92.36±3.33)分、护理文书(90.33±4.38)分,对照组分别为(85.45±8.66)分、(85.46±1.24)分、(80.69±5.36)分、(80.33±3.66)分、(85.45±4.33)分,观察组评分明显高于对照组,组间对比,差异有统计学意义(t=4.603 3、18.600 0、8.732 6、12.758 5、4.153 9,P<0.05)。观察组心理功能(67.78±11.22)分、躯体功能(74.22±10.25)分、物质生活(75.22±6.55)分、社会功能(68.55±12.22)分;对照组分别为(55.55±10.22)分、(65.55±9.25)分、(66.33±6.55)分、(58.78±12.22)分;组间对比,差异有统计学意义(t=4.228 8、3.295 6、5.031 9、3.210 2,P<0.05)。观察组导管感染、血栓发生率明显低于对照组,护理满意度为96.2%,组间对比,差异有统计学意义(χ~2=3.887 9,P<0.05)。结论在血液透析患者临床护理措施中给予分层护理干预效果显著,值得临床应用推广。

Objective To explore the application value of stratified nursing in clinical nursing of hemodialysis patients.Methods A total of 55 hemodialysis patients received from our hospital from December 2016 to July 2018 were selected as baseline data.According to different nursing measures,they were divided into two groups according to the ratio of 1:1,and the control group(n=28)and the observation group(n=27),the patients who received routine care were the control group,and the patients who underwent stratified nursing intervention were the observation group.The quality of care,quality of life,hemodialysis catheter infection,thrombosis rate and nursing satisfaction were compared between the two groups.Results The observation group had basic nursing(93.65±3.66)points,professional skills(92.46±1.53)points,nursing safety(92.36±4.53)points,ward management(92.36±3.33)points,nursing documents(90.33±4.38)points,and the control group were(85.45±8.66)points,(85.46±1.24)points,(80.69±5.36)points,(80.33±3.66)points,(85.45±4.33)points points,the observation group score was significantly higher than the control group,the comparison between groups,the difference was significant(t=4.603 3,18.600 0,8.732 6,12.758 5,4.153 9,P<0.05),respectively.The psychological function of the observation group(67.78±11.22)points,physical function(74.22±10.25)points,material life(75.22±6.55)points,and social function(68.55±12.22)points;the control group were(55.55±10.22)points,(65.55±9.25)points,(66.33±6.55)points,(58.78±12.22)points;significant differences between groups,(t=4.228 8,3.295 6,5.031 9,3.210 2).The incidence of catheter infection and thrombosis in the observation group was significantly lower than that in the control group,and the satisfaction degree of nursing was 96.2%.The difference between groups was significant(χ^2=3.887 9,P<0.05).Conclusion The effect of stratified nursing intervention in clinical nursing measures of hemodialysis patients is significant,which is worthy of clinical application.
