16排低剂量多层螺旋CT在肺部孤立实性小癌结节灶诊断中的应用价值 被引量:16

摘要 目的探讨16排低剂量多层螺旋CT肺部孤立实性小结节在肺癌早诊早治中的应用价值。方法回顾分析我院经胸腔镜手术证实42例肺部孤立实性小结节癌灶薄层低剂量螺旋CT影像表现,提高对实性小肺癌结节病灶形态特征的认识,为肺癌早诊早治提供影像诊断依据。结果 42例病灶均为肺内单一孤立实性小结节灶,其中35例形态欠规整,伴有浅小分叶改变,密度均匀;15例伴有毛刺、胸膜牵拉;8例伴含气小空泡;21例伴周围血管聚集。除发现肺内单发孤立结节病灶外,其余肺组织内未见异常征象,无肺门及纵隔淋巴结肿大及胸廓骨质破坏吸收等。全部病灶均经胸腔镜手术切除并经病理证实为癌性小结节灶。结论在肺部孤立实性小癌结节早期诊断治疗中,16排低剂量多层螺旋CT肺部扫描可早期发现病灶,并依其形态学特征提供诊断信息,在肺癌筛查或临床检查中有效降低辐射剂量,为肺癌早诊早治提供有效术前诊断依据,值得临床推广。 Objective To investigate the value of 16 rows of low dose multi-slice spiral CT pulmonary isolated solid nodules in early diagnosis and treatment of lung cancer.Methods Retrospectively analyze 42 cases with pulmonary isolated solid nodule carcinoma with thin layer low dose spiral CT images confirmed by thoracoscopy in our hospital,to improve the understanding of the morphological characteristics of solid small lung cancer nodule lesions,and to provide imaging diagnostic basis for early diagnosis and treatment of lung cancer.Results Forty-two cases were all single isolated solid nodes in lung,of which 35 were poorly regulated,with shallow-leaf changes and uniform density;15 cases with burr,pleural pull;8 cases with small cavitation;21 cases with peripheral vascular aggregation.In addition to isolated nodule lesions in the lungs,there were no abnormal signs in the remaining lung tissue,no pulmonary portal and mediastinal lymph node enlargement and thoracic bone destruction absorption.All the lesions were removed by thoracoscopic surgery and were found to be cancerous focal points by pathology.Conclusion Sixteen rows of low-dose multi-slice spiral CT lung scan can detect the lesion early in the early diagnosis and treatment of pulmonary isolated solid nodules,and provide diagnostic information according to its morphological characteristics,and effectively reduce the radiation dose in lung cancer screening or clinical examination.To provide the effective preoperative diagnosis basis for early diagnosis and treatment of lung cancer,it is worthy of clinical promotion.
