Solitaire支架机械取栓与静脉溶栓治疗急性脑梗死的临床对比分析

Clinical comparison of Solitaire stent mechanical thrombectomy and intravenous thrombolysis for acute cerebral infarction

目的研究比较Solitaire支架机械取栓与静脉溶栓治疗急性脑梗死的临床疗效。方法选取本院2016年10月至2018年10月收治的60例急性脑梗死患者为研究对象,按数字奇偶法分为A组与B组(各30例),A组给予Solitaire支架机械取栓治疗,B组给予静脉溶栓治疗,比较两组临床治疗效果。结果两组分别经治疗后,A组总有效率(86.67%)显著高于B组(60.00%)(P<0.05);治疗前,两组NIHSS评分比较差异无统计学意义;治疗后1 h、24 h及1周,两组NIHSS评分显著低于治疗前(P<0.05),A组治疗后24 h及1周NIHSS评分显著低于B组(P<0.05);A组消化道出血、脑出血、尿道出血及牙龈出血总发生率(10.00%)显著低于B组(33.33%)(P<0.05)。结论 Solitaire支架机械取栓较静脉溶栓更有利于提高血管再通率,改善患者神经功能,减少出血发生率,值得临床推广实施。

Objective To compare the clinical efficacy of Solitaire stent mechanical thrombectomy and venous thrombolysis in the treatment of acute cerebral infarction. Methods Sixty patients with acute cerebral infarction admitted to our hospital from October 2016 to October 2018 were enrolled in the study. They were divided into group A and group B (40 cases each) according to the numerical parity method. Group A was given Solitaire stent mechanical extraction. Suppository treatment, group B was given intravenous thrombolytic therapy, and the clinical effects of the two groups were compared. Results After treatment, the total effective rate of group A (86.67%) was significantly higher than that of group B (60.00%)(P<0.05). There was no significant difference in NIHSS score between the two groups before treatment. The NIHSS scores of the two groups were significantly lower than those before treatment at 1 h, 24 h and 1 week after treatment (P<0.05). The NIHSS scores of group A were significantly lower than those of group B at 24 h and 1 week after treatment (P<0.05). The total incidence of bleeding, cerebral hemorrhage, urethral hemorrhage and gingival bleeding (10.00%) was significantly lower than that of group B (33.33%)(P<0.05). Conclusion Solitaire stent mechanical thrombectomy is more beneficial to improve the recanalization rate of blood vessels, improve the neurological function of patients and reduce the incidence of bleeding. It is worthy of clinical promotion.
