神经内镜与显微镜下经蝶窦手术治疗垂体瘤的效果比较 被引量:5

摘要 目的比较神经内镜与显微镜下经蝶窦手术治疗垂体瘤的效果。方法回顾性分析2015年3月至2018年3月在我院接受经蝶窦手术治疗的58例垂体瘤患者的临床资料,将采用神经内镜下手术的30例患者纳入神经内镜组,将采用显微镜下手术的28例患者纳入显微镜组。比较两组患者的手术相关情况、治疗效果和并发症发生情况。结果神经内镜组的手术时间和住院时间短于显微镜组,术中出血量少于显微镜组(P<0.05);神经内镜组患者的治疗总有效率高于显微镜组,但组间无显著差异(P>0.05);神经内镜组并发症总发生率低于显微镜组(P<0.05)。结论采用神经内镜或显微镜经蝶窦手术治疗垂体瘤的效果相近,但采用神经内镜治疗垂体瘤的手术时间更短,术中出血量更少,患者恢复更快,并发症更少,临床价值更高。 Objective To compare the curative effect of neuroendoscopic and microscopic transsphenoidal surgery for pituitary adenomas. Methods Theclinical data of 58 patients who underwent transsphenoidal surgery in our hospital from March 2015 to March 2018 were retrospectively analyzed. Thirty patients who underwent neuroendoscopic surgery were included in neuroendoscopic group, and twenty-eight patients who underwent microscopic surgery were included in microscopic group. The operative correlations, curative effects and complications of the two groups were compared. Results The operation time and hospitalization time in the neuroendoscopic group were shorter than those in the microscopic group,and the intraoperative bleeding volume was less than that in the microscopic group(P<0.05). The total curative effective rate in the neuroendoscopic group was higher than that in the microscopic group, but there was no significant difference between the two groups(P>0.05). The total incidence of complications in the neuroendoscopic group was lower than that in the microscopic group(P <0.05). Conclusion Neuroendoscopic and microscopic transsphenoidal surgery for pituitary adenomas has similar effect, but neuroendoscopic surgery for pituitary adenoma has shorter operation time, less bleeding,faster recovery, fewer complications, and higher clinical value.
