马来酸麦角新碱联合垂体后叶注射液治疗宫缩乏力性产后出血的临床效果 被引量:8

摘要 目的研究马来酸麦角新碱联合垂体后叶注射液治疗宫缩乏力性产后出血的临床效果。方法选取2017年1月至2018年6月我院收治的宫缩乏力性产后出血患者76例,根据随机数字表法将其分为研究组(38例)和对照组(38例)。对照组于分娩后静脉滴注垂体后叶注射液治疗,研究组在此基础上给予肌肉注射马来酸麦角新碱注射液治疗。比较两组患者治疗前、后的血清炎症因子水平、应激反应指标、凝血指标及临床疗效。结果治疗后,两组患者的IL-6、TNF-α、ICAM1、NE、E、ANG-Ⅱ、ALD水平均显著降低,且研究组低于对照组(P<0.05);治疗后,两组患者的FIB水平均降低,PT、APTT均延长,且研究组均优于对照组(P<0.05);研究组患者的治疗总有效率为92.11%,显著高于对照组的73.68%(P<0.05)。结论马来酸麦角新碱联合垂体后叶注射液治疗宫缩乏力性产后出血可缩短出血时间,减少出血量,有效改善患者机体炎症反应及应激反应,值得临床推广使用。 Objective To study the effect of ergometrine maleate injection combined with posterior pituitary injection in the treatment of postpartum hemorrhage due to uterine inertia. Methods A total of 76 patients with postpartum hemorrhage due to uterine inertia admitted in our hospital from January 2017 to June 2018 were selected. The patients were divided into study group(38 cases) and control group(38 cases) according to the random number table method. The control group was treated with intravenous infusion of posterior pituitary injection after delivery. On this basis, the study group was treated with intramuscular injection of ergometrine maleate injection. The levels of serum inflammatory factors, stress response indexes, coagulation indexes before and after treatment and clinical efficacy between the two groups were compared. Results After treatment, the levels of IL-6, TNF-α, ICAM1, NE, E, ANG-Ⅱ and ALD in the two groups decreased, and those in the study group were lower than the control group(P<0.05). After treatment, the levels of FIB in both groups decreased, the PT and APTT prolonged, and those in the study group were better than the control group(P<0.05).The total effective rate of treatment in the study group was 92.11%, which was significantly higher than 73.68% in the control group(P<0.05). Conclusion The ergometrine maleate injection combined with posterior pituitary injection in the treatment of postpartum hemorrhage due to uterine inertia can shorten the bleeding time, reduce the amount of bleeding and effectively improve the patient’s inflammatory response and stress response, which is worthy of clinical popularization and application.
