缬沙坦单药及伍用中药治疗慢性肾小球肾炎疗效及安全性比较

Comparing the efficacy and safety of simple valsartan and combined traditional Chinese medicine in chronic glomerulonephritis

目的比较缬沙坦单药及伍用中药治疗慢性肾小球肾炎的临床疗效及安全性。方法 120例慢性肾小球肾炎病人随机分为单药组和联合组,单药组仅口服缬沙坦,联合组同时口服缬沙坦和黄葵。治疗8周后评价治疗有效率和安全性,比较治疗前后患者血肌酐、尿素氮和24 h尿蛋白含量。结果联合组治疗慢性肾小球肾炎总体有效率(90.0%)显著高于单药组(63.3%)(χ~2=11.93,P<0.01);两组患者治疗后血肌酐、尿素氮和24 h尿蛋白含量均较治疗前显著改善(P均<0.05),且联合组患者改善更为明显。两组患者不良反应发生率差异无统计学意义(校正χ~2=0,P>0.05)。结论缬沙坦伍用黄葵治疗慢性肾小球肾炎效果显著高于缬沙坦单药治疗,且不良反应与单用无显著差异。缬沙坦伍用黄葵治疗慢性肾小球肾炎安全、高效,值得临床推广应用。

Objective To compare the efficacy and safety of simple valsartan and combined traditional Chinese medicine in the treatment of chronic glomerulonephritis. Methods One hundred and twenty patients with chronic glomerulonephritis were recruited and randomized into simple and combined medication group. Patients in the simple medication group were treated with oral valsartan, and those in combined group were managed with oral valsartan plus Huangkui capsule. Then the two groups were evaluated on the effective rate and safety after 8 weeks of medication, and compared concerning the serum creatinine and urea nitrogen levels as well as 24 h urinary protein content before and after therapy. Results The overall response rate was significantly higher in combined medication group than in the simple medication group(90% vs. 63.3%,χ^2 = 11.93, P <0.01), and the serum creatinine and urea nitrogen levels as well as 24 h urinary protein content were significantly improved after treatment( P < 0.05 ), and improvement was dominant in patients treated with combined regimen. The adverse effect remained insignificant between groups(χ^ 2 = 11.93, P >0.01). Conclusion Combined valsartan with Huangkui capsule can be superior to simple valsartan regimen in safety and efficacy for chronic glomerulonephritis, and is worthy of wider clinical recommendation.
