高压氧治疗重型颅脑损伤对患者术后肢体功能及神经功能的影响研究 被引量:25

摘要 目的探究高压氧治疗重型颅脑损伤对患者术后肢体功能及神经功能的影响,为重型颅脑损伤的治疗提供参考。方法选取手术治疗的重型颅脑损伤患者66例为观察对象,随机分为高压氧组与对照组,每组33例。对照组给予患者术后传统常规治疗,高压氧组术后给予高压氧治疗,采用美国国立卫生研究院卒中量表(NIHSS)评价2组患者的神经功能,应用Fugl-Meyer运动功能评分法评估2组患者的肢体功能。并比较2组患者临床疗效。结果治疗前,2组患者的上肢功能评分、下肢功能评分以及Fugl-Meyer总分比较差异无统计学意义(P>0.05);治疗后,2组患者以上评分有所提高且高压氧组明显高于对照组,差异有统计学意义(P<0.05);高压氧组(93.94%)与对照组(69.70%)的治疗总有效率比较差异有统计学意义(P<0.05);治疗前,2组患者的NIHSS评分比较无差异(P>0.05);治疗后2组NIHSS评分降低且高压氧组明显低于对照组(P<0.05)。结论高压氧治疗重型颅脑损伤,能够明显改善患者术后肢体功能、神经功能,疗效确切,对于促进患者术后的恢复有积极作用,具有较高的临床应用价值。 Objective To explore the effects of hyperbaric oxygen on limb function and nerve function in patients with severe craniocerebral injury after operation,in order to provide reference for the treatment of severe craniocerebral injury. Methods A total of 66 patients with severe craniocerebral injury were randomly divided into hyperbaric oxygen group ( n =33) and control group ( n =33).The patients in control group were treated by conventional therapy after operation,however,the patients in hyperbaric oxygen group were treated by hyperbaric oxygen after operation.The neurologic function of patients in both groups was evaluated by NIHSS (National Institutes of Health Stroke scale).The limb function of patients in both groups was evaluated by Fugl-Meyer motor function score.The clinical curative effects were observed and compared between the two groups. Results Before treatment,there were no significant differences in upper limb function scores,lower limb function scores and total Fugl-Meyer scores between the two groups ( P >0.05).After treatment the scores mentioned above were significantly increased in both groups,moreover,which hyperbaric oxygen group were significantly higher than those in control group ( P <0.05).In addition the total effective rate in hyperbaric oxygen group was significantly higher than that in control group (93.94% vs 69.70%, P <0.05).Before treatment there were no significant differences in NIHSS scores between the two groups ( P >0.05),however, after treatment the NIHSS scores were significantly decreased in both groups,moreover,which hyperbaric oxygen group were significantly lower than those in control group ( P <0.05). Conclusion Hyperbaric oxygen therapy for severe craniocerebral injury can obviously improve the limb function and nerve function of patients after operation,which has positive effects on the recovery of patients after operation,therefor, which has important clinical application value.
