摘要
目的:观察脑囊虫病伴发癫痫患者术后脑脊液一氧化氮(NO)、超氧化物歧化酶(SOD)、丙二醛(MDA)水平变化情况,分析术后脑脊液NO、SOD、MDA水平与患者预后相关性。方法:回顾性分析48例脑囊虫病伴发癫痫患者临床资料。均行手术治疗,术后1个月时评估近期疗效,根据其近期疗效评估结果将其分为预后良好组(研究组,n=41)和预后不良组(对照组,n=7)。比较患者术中及术后3d时脑脊液NO、SOD、MDA水平差异,分析术后3d时脑脊液NO、SOD、MDA水平与其近期预后的相关性。结果:术后3d时,48例患者脑脊液NO、MDA水平均较术中时降低,SOD水平均较术中时升高(P<0.05),且研究组脑脊液NO、MDA水平低于对照组,SOD水平高于对照组(P<0.05)。Logistics回归分析表明,术后3d时脑脊液NO、MDA水平与脑囊虫病患者近期预后不良呈正相关性,SOD水平呈负相关性(P<0.05)。结论:行手术治疗后脑囊虫病伴发癫痫患者脑脊液NO、SOD、MDA水平显著改善,术后脑脊液NO、SOD、MDA水平与患者预后密切相关。
Objective:To observe the changes of levels of nitric oxide (NO),superoxide dismutase (SOD) and malondialdehyde (MDA) in cerebrospinal fluid in patients with cerebral cysticercosis complicated with epilepsy,and analyze the correlation between levels of NO,SOD and MDA in postoperative cerebrospinal fluid and prognosis.Methods:The clinical data of 48 patients with cerebral cysticercosis complicated with epilepsy were retrospectively analyzed.All patients were given surgery,and the short-term efficacy was evaluated at 1 month after operation.According to the results of short-term efficacy evaluation,they were divided into good prognosis group (study group,n =41) and poor prognosis group (control group,n =7).The levels of NO,SOD and MDA in cerebrospinal fluid were compared during operation and at 3 days after operation.The correlation between the levels of NO,SOD and MDA in cerebrospinal fluid and the short-term prognosis was analyzed at 3 days after operation.Results:At 3 days after operation,the levels of NO and MDA in cerebrospinal fluid were decreased compared with those during operation,and the SOD level was increased compared with that during operation ( P <0.05).At 3 days after operation,the levels of NO and MDA in cerebrospinal fluid in study group were lower than those in control group while the SOD level was higher than that in control group ( P <0.05).Logistics regression analysis showed that the levels of NO and MDA in cerebrospinal fluid at 3 days after operation were positively correlated with the poor short-term prognosis of patients with cerebral cysticercosis,and the level of SOD was negatively correlated with poor prognosis ( P <0.05).Conclusion:The levels of NO,SOD and MDA in cerebrospinal fluid are significantly improved in patients with cerebral cysticercosis and epilepsy after surgery.The levels of NO,SOD and MDA in postoperative cerebrospinal fluid are closely related to the prognosis of patients.
