高位虚挂线引流与减压引流法在高位肛周脓肿切除术中的应用效果比较

Comparing high-position virtual thread drainage and decompression drainage in treating high perianal abscess

目的比较高位虚挂线引流与减压引流法在高位肛周脓肿切除术中的应用效果。方法纳入2016年1月至2018年1月沧州市人民医院胃肠微创外科收治的76例高位肛周脓肿患者为研究对象,按入院先后顺序将患者分为高位虚挂线引流组和减压引流组,每组各纳入38例,患者均接受肛周脓肿切除术。比较两组手术相关指标、术后第2 d及第7 d的疼痛程度、术后6个月内的肛肠直肠功能、脓肿复发率以及肛瘘形成率。结果两组手术时间比较,差异无统计学意义(P> 0.05),减压引流组术后住院时间、伤口愈合时间短于高位虚挂线引流组,差异均有统计学意义(均P <0.05)。减压引流组术后第2 d疼痛情况轻于高位虚挂线引流组,差异有统计学意义(P <0.05);两组术后第7 d疼痛情况比较,差异无统计学意义(P> 0.05)。两组术后6个月肛门功能均正常,未见出现漏液、漏气等情况。两组肛周脓肿复发率及肛瘘形成率比较,差异均无统计学意义(均P> 0.05)。结论与高位虚挂线引流比较,高位肛周脓肿切除术后减压引流更有助于促进患者术后早期恢复、减轻术后早期疼痛。

Objective To compare effectiveness of high-position virtual thread drainage and decompression drainage in treating high perianal abscess.Methods 76 patients with high perianal abscess who were treated in Department of Gastrointestinal Minimally Invasive Surgery,Cangzhou People’s Hospital,between January 2016 and January 2018 were recruited as study subjects.Patients were assigned to control group(received high-position virtual thread drainage)and treatment group(received decompression drainage),with 38 patients in each group,group based on sequence of admission.Both groups of patients with incision of perianal abscess.Indices related to operation,pain at 2nd and 7th day after operation,anal and rectal function in 6 months after surgery,recurrence of abscess and formation of fistula were compared between the two groups.Results There was no significant difference in duration of operation between the two groups(P>0.05).Duration of postoperative hospitalization and time to wound healing were significantly shorter in treatment group than in control group(P<0.05).Pain at 2nd day after operation was significantly less in control group than in treatment group(P<0.05)and pain at 7th day was comparable between the two groups(P>0.05).Anal function was normal in both groups in 6 months after operation and no patient had leakage(fluid or gas).There was no difference between the two groups in recurrence of perianal abscess and formation of fistula(P>0.05).Conclusion Compared with high-position virtual thread drainage,decompression drainage after incision of high perianal abscess was more beneficial in promoting postoperative recovery and alleviating early pain.
