应变率成像技术评价风湿性心脏病二尖瓣狭窄患者的左房功能 被引量:3

摘要 目的:探讨应变率成像技术(SRI)评价风湿性心脏病二尖瓣狭窄(MS)患者左房功能的应用价值。方法:选择2012年2月至2017年9月在广西医科大学第二附属医院收治的风湿性心脏病MS患者96例,按MS程度分为轻度组、中度组和重度组,同期选取健康志愿者40例作为正常组。根据超声心电图记录左房容积参数:左房最大容积(LAVmax)、左房最小容积(LAVmin)、左房P容积(LAVp)、左房主动射血容积(LAAEV)、左房主动射血分数(LAAEF)、左房被动射血容积(LAPEV)、左房被动射血分数(LAPEF)。测量各组左房各壁收缩期应变率峰值(SRs)、舒张早期应变率峰值(SRe)和舒张晚期应变率峰值(SRa),并计算SRs、SRe、SRa的平均值mSRs、mSRe、mSRa。结果:与正常组比较,轻度组、中度组和重度组LAVmin升高,LAPEF、LAAEF、mSRs、mSRe、mSRa明显降低,且随病情加重呈逐渐升高/下降趋势,差异均有统计学意义(均P<0.05)。结论:SRI可用于评价风湿性心脏病MS患者的左房功能。 Objective: To investigate the clinical value of strain rate imaging (SRI) in evaluating left atrial function in patients with mitral stenosis (MS) rheumatic heart disease. Methods: A total of 96 patients with MS rheumatic heart disease and 40 healthy controls were selected. The left atrial maximum volume (LAVmax),left atrial minimum volume (LAVmin),left atrial P volume (LAVp),left atrial active ejection volume (LAAEV),left atrial active ejection fraction (LAAEF),left atrial passive ejection volume (LAPEV) and left atrial passive ejection fraction (LAPEF) were measured by UCG. Left atrial peak systolic strain rate (SRs),peak early diastolic strain rate (SRe) and peak advanced diastolic strain rate (SRa) on each wall of the left atrium were recorded. Results: Compared with the healthy controls,the LAVmin was significantly increased,while the LAPEF,LAAEF,mSRs,mSRe and mSRa were decreased in patients with MS,and these indexes changed with the aggravation of the disease (P<0.05). Conclusion: SRI could be used to evaluate the left atrial function of patients with MS rheumatic heart disease.
