1例肾上皮样血管平滑肌脂肪瘤的诊断

Diagnosis of one case of renal epithelioid angiomyolipoma

目的探讨肾上皮样血管平滑肌脂肪瘤(EAML)的有效诊断方法。方法对1例肾EAML患者的临床资料、影像学及病理检查结果作回顾性分析。结果患者主诉为右侧腰部胀痛3 d,查体及心电图、X线、血常规等检查均无异常发现,腹部CT检查提示右肾肿块,全麻下行开放性右肾根治性切除术,切除肿瘤经术后病理检查诊断为右肾EAML。结论肾EAML缺乏特异性的临床表现,影像学检查与其他肾肿瘤难以鉴别,确诊需要依靠病理形态学检查结合免疫组化染色检查。

Objective To explore the effective diagnostic method of renal epithelioid angiomyolipomas( EAML).Methods The clinical data,imaging examination and pathologic features of 1 case of renal EAML were retrospectively analyzed. Results The patient complained of right lower back pain for 3 days,and the physical examination,electrocardiogram,X-ray,blood routine were normal. Abdominal computed tomography revealed a right renal mass;the radical resection of the right kidney tumor was done under general anesthesia,and the postoperative pathological diagnosis of right kidney was EAML. Conclusions The disease lacks of specificity of clinical manifestations,and the imaging findings with other renal tumor are difficult to identify. Pathology and immunohistochemistry are important to confirm the diagnosis.
