多学科团队模式在糖尿病患者管理中的应用

目的分析临床对糖尿病者采用多学科团队模式行疾病管理,患者疾病控制情况的效果。方法选择2016年4月—2017年4月医院收治的2型糖尿病者120例,按数字表法进行随机分组,各60例。对照组(行常规护理)、观察组(在治未病指导下行多学科团队疾病管理),治疗结束后随访1年,观察护理前、出院1年后患者自我管理能力、血糖控制、生存质量,统计并发症、低血糖发生等情况。结果观察组并发症发生率低、低血糖发生少,P<0.05;观察组出院后1年血糖水平、生存质量评分低于对照组,自我管理评分高于对照组,P<0.05。结论通过对DM者以治末病思想指导进行多学科团队疾病管理,患者血糖得到有效控制,并发症发生率明显降低。

Objective To analyze the effect of multidisciplinary team mode on disease management and disease control in diabetic patients.Methods 120 patients with type 2 diabetes admitted to hospital from April 2016 to April 2017 were taken as the objects and randomly divided into control group of 60 cases(applied routine nursing)and observation group of 60 cases(applied multidisciplinary team disease management under the guidance of pre-treatment).The patients were followed-up for one year after treatment.Patients'self-management ability,blood sugar control,quality of life,complications and hypoglycemia,etc.before nursing and one-year-hospital-discharge were observed.Results The incidence of complications and hypoglycemia in the observation group was low(P<0.05),the blood sugar level and quality of life score in the observation group were lower than those in the control group one year after discharge,and the self-management score in the observation group was higher than that in the control group(P<0.05).Conclusion Through multi-disciplinary team disease management guided by the idea of end-of-life treatment for DM patients,blood sugar is effectively controlled,and the incidence of complications is significantly reduced.
