腹腔镜肝囊肿开窗术41例治疗体会

Experience of laparoscopic liver cyst window opening in 41 cases

目的探讨腹腔镜肝囊肿开窗术的临床疗效。方法回顾性分析我科2014年1月至2018年8月收治的41例肝囊肿行腹腔镜肝囊肿开窗术的资料。结果 41例患者全部行腹腔镜手术,40例未出现胆漏,腹腔感染,出血等严重并发症,术后1例出现胆漏,经过通畅引流1周后胆漏自行愈合,无死亡病例,随访3个月～1年,36例取得治愈效果,囊肿消失,5例复发。结论腹腔镜肝囊肿开窗引流术具有疗效确切、创伤小、恢复快的特点,掌握肝囊肿的腹腔镜手术指征,术中根据囊肿的特点实施正确的操作方法是减少并发症及复发率的关键。

Objective To investigate the clinical effect of laparoscopic hepatic cyst window opening. Methods the data of 41 cases of hepatic cysts a from January 2014 to August 2018 were retrospectively analyzed. Results All 41 patients underwent laparoscopic surgery, 40 patients did not have serious complications such as bile leakage, abdominal infection, and hemorrhage. After the operation, 1 patient had bile leakage. After 1 week of drainage, the bile leakage healed itself, and there were no deaths. Follow up 3 months to 1 year, 36 cases achieved cure effect. The cysts disappeared, five cases recurred. Conclusion laparoscopic open window drainage of liver cyst has the characteristics of accurate curative effect, small trauma, and rapid recovery. To master the indication of laparoscopic operation of liver cyst, it is the key to reduce the complication and recurrence rate to implement the correct operation method according to the characteristics of cyst.
