经心尖经导管主动脉瓣置换术的麻醉处理 被引量:1

摘要 主动脉瓣置换的传统手术方式为体外循环辅助下经胸骨正中切口。对于高龄和(或)伴有糖尿病、肾功能不全等疾病的患者,常常由于全身情况不佳而无法接受传统手术。据报道,有高达30%~40%的主动脉瓣病变患者因不能耐受手术退而接受药物治疗,严重影响其生活质量[1]。经导管主动脉瓣置换术(TAVI)为近年出现的新颖手术方式,主要通过大动脉(股动脉、锁骨下动脉、腋动脉)途径实施,因创伤小、术后恢复快受到外科医师和高龄患者的青睐。相比于经大动脉途径,经心尖TAVI操作更为直接,可以克服外周血管条件不良等障碍,具有经大动脉途径所不具备的优势[2].
