米索前列醇不同用药途径对经阴道自然分娩产后出血的预防效果

Preventive effect of misoprostol on postpartum hemorrhage in spontaneous vaginal delivery

目的观察米索前列醇不同用药途径对经引导自然分娩产后出血产妇的预防效果。方法选取医院进行诊疗的产妇300例为研究对象,随机分为A、B、C组,每组100例。对产妇施以不同的米索前列醇给药方式,A组口服给药,B组直肠给药,C组阴道给药。A组在胎儿头部着冠时口服,B组和C组均在胎儿正常阴道自然分娩后,对产妇的直肠以及阴道后穹窿处放置米索前列醇药物。比较3组第三产程时间、产后出血量以及用药不良反应发生情况。结果经过不同的米索前列醇给药方式,产妇的第三产程时长、产后出血量以及出血症状的发生和药物不良反应发生率均无显著差异(P> 0. 05)。结论经阴道自然分娩的产妇施以不同给药方式的米索前列醇药物,产妇的产后出血预防成效无显著差异。但由于在对产妇施以阴道给药方式时,药物可能出现稀释或者冲出现象,因此在临床医学中通常使用口服或者直肠用药方式。

Objective To observe the preventive effect of misoprostol on postpartum hemorrhage after natural delivery.Methods 300 parturients were randomly divided into groups A,B and C,with 100 cases in each group.Group A was given orally,group B was given rectally and group C was given vaginally.Group A was given orally when the fetal head was crowned.Group B and Group C were given misoprostol in the rectum and posterior vaginal fornix after normal vaginal delivery.The time of the third stage of labor,the amount of postpartum hemorrhage and the occurrence of adverse drug reactions were compared among the three groups.Results There were no significant differences in the duration of the third stage of labor,the amount of postpartum hemorrhage,the occurrence of bleeding symptoms and the incidence of adverse drug reactions among the three groups( P > 0.05).Conclusion There was no significant difference in the preventive effect of misoprostol on postpartum hemorrhage among different administration modes.However,due to the possibility of dilution or washout of vaginal administration,oral or rectal administration is usually used in clinical practice.
