单双侧钢板内固定术治疗复杂胫骨平台骨折疗效比较

Comparison of efficacy of unilateral or bilateral steel plate internal fixation in treating complex tibial plateau fracture

目的比较单、双侧钢板内固定术治疗复杂胫骨平台骨折的临床疗效。方法对2013年2月-2018年2月收治的80例复杂胫骨平台骨折患者,按照随机数字表法分为观察组(40例),对照组(40例),观察组给予双侧钢板内固定术治疗,对照组给予单侧钢板内固定术治疗。治疗结束后,比较两组手术时间、手术出血量、住院时间、骨折愈合时间,以及临床疗效和并发症。结果两组在手术出血量上比较无统计学意义;观察组手术时间显著高于对照组;住院时间和骨折愈合时间显著低于对照组(P <0.05);观察组术后并发症发生率为12.5%,显著低于对照组的35%;观察组治疗有效率为75%,显著高于对照组的42.5%,差异均具有统计学意义(P<0.05)。结论双侧钢板内固定术较单侧钢板内固定术在治疗复杂胫骨平台骨折上具有更高的临床疗效,以及更低的并发症发生率,值得临床推广应用。

Objective To investigate the clinical efficacy of unilateral or bilateral steel plate internal fixation in the treatment of complex tibial plateau fracture. Methods Total of 80 patients with complex tibial plateau fractures were allocated in our study, and divided into two groups: study group(40 cases),control group(40 cases). The control group was treated with unilateral steel plate internal fixation, the study group was treated with bilateral steel plate internal fixation. After treated, the operation time,operative blood loss, hospital stay, fracture healing time, clinical curative effect and incidence were compared between two groups. Results The operative blood loss of two groups was similar(P>0.05),and the operation time in study group were longer than the time in control group, the hospital stay and fracture healing time in study group were significantly shorter than that in control group. The postoperative complication rate in study group was 12.5%, significantly lower than that of 35% in the control group. The clinical effective rate was 75% in study group, which was significantly higher than that of 42.5% in control group. Conclusion Bilateral steel plate internal fixation in the treatment of complex tibial plateau fractures is effective than unilateral steel plate internal fixation.
