超声影像评价矮小症男童睾丸的初步临床研究 被引量:2

摘要 目的探讨超声影像在矮小症男童睾丸大小评估中的价值。方法选取南京医科大学附属上海一院临床医学院2015年6月至2017年12月期间就诊并进行阴囊内容物二维超声检查的男童共218例,其中76例矮小症男童为矮小症组,年龄7~13岁;对照组为142例健康男童,年龄7~13岁。超声检查内容包括睾丸形态、大小、包膜、内部回声、血供情况,评价睾丸大小的定量指标为睾丸体积平均值(MVT),并比较两组受检者的超声检测结果。结果所有受检者均成功完成睾丸的超声检查;矮小症组男童睾丸的形态、包膜、内部回声、血供与健康男童差异无统计学意义;7~13岁各年龄组矮小症男童的平均MVT分别为(0.47±0.13) mL、(0.84±0.20) mL、(0.80±0.30) mL、(0.78±0.24) mL、(0.67±0.24) mL、(1.17±0.33) m L、(1.04±0.28) mL,明显低于健康男童组的(0.91±0.35) mL、(2.20±0.25) mL、(1.93±0.29) m L、(2.82±0.54) m L、(4.03±1.07) m L、(3.96±0.88) m L、(4.50±1.46) m L,差异均有统计学意义(P<0.05)。结论利用超声影像可以评价矮小症男童睾丸体积的变化;同时,超声影像检查无创、无辐射患儿容易接受,临床应用前景良好。 Objective To evaluate the diagnostic value of ultrasonography in evaluation of testis in boys with short stature. Methods A total of 218 boys were selected from the Department of Medical Ultrasound, Shanghai General Hospital of Nanjing Medical University from June 2015 to December 2017 for two-dimensional ultrasound examination of scrotal contents, including 142 healthy boys(the control group, aged 7-13 years) and 76 boys with short stature(the study group, aged 7-13 years). Testicular morphology, size, capsule, internal echo, and blood supply were measured by ultrasonography. Mean volume of testes(MVT) was calculated and used as the quantitative index of testicular size.The results of two groups were compared. Results No statistically significant difference was observed between short stature boys and healthy boys in testicular morphology, capsule, internal echo, and blood supply. But the MVT of short stature boys aged 7, 8, 9, 10, 11, 12, 13 years were(0.47±0.13) m L,(0.84±0.20) m L,(0.80±0.30) m L,(0.78±0.24) m L,(0.67±0.24) m L,(1.17±0.33) m L,(1.04±0.28) m L, significantly smaller than(0.91±0.35) m L,(2.20±0.25) m L,(1.93±0.29) m L,(2.82±0.54) m L,(4.03±1.07) m L,(3.96±0.88) m L,(4.50±1.46) m L of healthy boys(P<0.05). Conclusion Ultrasonography can be used to evaluate the changes of testicular volume in short stature boys. And as a non-invasive,non-radiation imaging examination method, ultrasonography is easy to be accepted and have a good prospect in clinic.
