摘要
目的 :探讨内皮细胞特异分子-1(endothelial cell specific molecule-1,Endocan)、脂肪特异性丝氨酸蛋白酶抑制剂(visceral adipose tissue-derived serine protease inhibitor,Vaspin)水平对微血管性心绞痛诊断的临床价值。方法 :该研究共纳入微血管性心绞痛患者40例(MVA组),不稳定性心绞痛患者103例(UA组),同期于我院体检的正常人35例(正常对照组)。应用酶联免疫吸附试验(ELISA)法检测各组血清Endocan、Vaspin水平。结果 :(1)与正常对照组相比,UA组和MVA组血清Endocan水平升高,血清Vaspin水平降低(P均<0.01);与UA组相比,MVA组血清Endocan水平低,Vaspin水平高(P均<0.01)。(2)Spearman相关性分析显示,UA组血清Endocan水平与Gensini评分呈正相关(r=0.516,P<0.001),Vaspin水平与Gensini评分呈负相关(r=-0.563,P<0.001)。受试者工作特征(ROC)曲线分析显示,Endocan、Vaspin诊断MVA的曲线下面积分别是0.816、0.796。结论 :血清Endocan、Vaspin水平可能与血管内皮功能障碍密切相关,有可能成为MVA诊断的新型标志物。
Objectives: To investigate the value of using serum endothelial cell specific molecule-1 (Endocan) and visceral adipose tissue-derived serine protease inhibitor (Vaspin) in the diagnosis of patients with microvascular angina (MVA). Methods: A total of 40 patients with MVA (MVA group), 103 patients with UA (UA group) and 35 cases of healthy subjects (control group) were enrolled in this study. Endocan and Vaspin were detected by ELISA. Results: Compared with the control group, the level of Endocan was significantly higher while the Vaspin level was significantly lower in patients with MVA and UA (all P<0.01). Compared with patients with UA, the level of Endocan was significantly lower while the Vaspin level was significantly higher in patients with MVA (all P<0.01). Spearman correlation analysis showed that the Gensini score was positively correlated with Endocan (r=0.516, P<0.001), while negatively correlated with Vaspin ( r=-0.563. P<0.001 ) in the UA group. ROC curve analysis showed that the area under curve of differentiating MVA and control groups with Endocan and Vaspin were 0.816, 0.796. Conclusion: The serum levels of Endocan and Vaspin are closely related with endothelial dysfunction and might be used as new biomarkers for the diagnosis of MVA.
