彩色多普勒超声检查颈动脉斑块与冠心病关系的临床价值分析 被引量:10

摘要 目的对彩色多普勒超声检查颈动脉斑块与冠心病关系的临床价值进行评价分析,为今后的临床诊疗工作提供有价值的参考依据。方法选择获得临床确诊的冠心病患者87例作为观察组,另选取同期健康体检者87例作为对照组,对两组均进行彩色多普勒超声检查,对比分析两组受试者的斑块积分、病理学分型、颈动脉内中膜厚度(IMT)。结果对照组中检出软斑块2例,硬斑块3例,混合斑块1例;观察组中检出软斑块10例硬斑块65例,混合斑块12例;两组软斑块、硬斑块、混合斑块的检出率比较差异均具有统计学意义(P<0.05)。对照组的斑块积分为(0.87±0.05)分,显著低于观察组的(1.98±0.54)分,差异具有统计学意义(P<0.05);对照组的IMT为(0.68±0.15)mm,显著低于观察组的(1.94±0.49)mm,差异具有统计学意义(P<0.05)。结论经彩色多普勒超声对颈动脉斑块进行检查,可为临床冠心病的诊断、分型等提供可靠的参考依据,临床价值显著,值得关注并推广。 Objective To evaluate and analyze the clinical value of color Doppler ultrasound in detecting the relationship between carotid plaque and coronary heart disease, and to provide valuable reference for future clinical diagnosis and treatment. Methods A total of 87 patients with clinically confirmed coronary heart disease were selected as the observation group, and 87 healthy people in the same period were selected as the control group. Both groups were examined by color Doppler ultrasonography. The plaque score, pathological type and carotid artery intima-media thickness(IMT) of the two groups were compared and analyzed. Results In the control group, there were 2 cases of soft plaque, 3 cases of hard plaque, and 1 case of mixed plaque. In the observation group, there were 10 cases of soft plaque, 65 cases of hard plaque, and 12 cases of mixed plaque. Both groups had statistically significant difference in detection rate of soft plaque, hard plaque and mixed plaque(P<0.05). The control group had significantly lower plaque score as(0.87±0.05) points than(1.98±0.54) points in the control group. Their difference was statistically significant(P<0.05). The control group had significantly lower IMT as(0.68±0.15) mm than(1.94±0.49) mm in the observation group, and the difference was statistically significant(P<0.05). Conclusion The detection of carotid plaque by color Doppler ultrasound can provide reliable reference for the diagnosis and classification of clinical coronary heart disease. It has significant clinical value and deserves attention and promotion.
